夹闭与栓塞治疗颅内动脉瘤对蛛网膜下腔出血患者脑血管痉挛及神经功能的影响

Effects of Clipping and Embolization on Cerebral Vasospasm and Nerve Function in Patients with Subarachnoid Hemorrhage

目的:探讨使用夹闭与栓塞治疗颅内动脉瘤伴蛛网膜下腔出血(Intracranial Aneurysms with Subarachnoid Hemorrhage,SAH)的效果。方法:选取2018年1月至2019年3月福建省立医院南院神经外科收治的颅内动脉瘤伴SAH患者150例作为研究对象,按照随机数字表法随机分成对照组与观察组,每组75例。对照组行开颅夹闭术治疗,观察组则行血管内介入栓塞术治疗,观察2组治疗效果。结果:术前、术后1 d、术后3 d及术后7 d观察组患者脑血管痉挛率均显著低于对照组,差异有统计学意义(P <0. 05);术后7 d,2组患者神经功能与睡眠质量评分均较术前降低,观察组评分降低幅度显著,2组比较差异有统计学意义(P <0. 05)。结论:针对颅内动脉瘤伴SAH患者,实施栓塞治疗的方法,可显著降低患者脑血管痉挛率,改善患者神经功能及睡眠质量。

Objective: To investigate the effects of occlusion and embolization in the treatment of Intracranial Aneurysms with Subarachnoid Hemorrhage(SAH). Methods: A total of 150 cases of intracranial aneurysm patients with SAH admitted to the neurosurgery department of fujian provincial hospital south hospital from January 2018 to March 2019 were selected as research objects.According to the numerical random table method,they were randomly divided into observation group and control group,with 75 cases in each group.The control group received craniotomy clipping,while the observation group received endovascular embolization,and the therapeutic effect of the two groups was observed. Results: The rates of cerebral vasospasm in the observation group were significantly lower than those in the control group before,1 day after,3 days after and 7 days after surgery,with statistically significant differences( P <0.05).On the 7th day after the operation,the neurological function and sleep quality scores of the 2 groups were decreased compared with those before the operation,and the score of the observation group was significantly decreased,and the difference between the 2 groups was statistically significant( P <0.05). Conclusion: For intracranial aneurysm patients with SAH,embolization therapy can significantly reduce the rate of cerebral vasospasm,improve the neurological function and sleep quality of patients.
