调查反馈机制在住院医师规范化培训工作中的应用效果

The application of investigation feedback mechanism in the standardized residency training

目的探究调查反馈机制在住院医师规范化培训(住培)中的应用效果。方法选取2014~2016级住培学员258人为研究对象,2016年6月至2017年6月期间,每半年对其进行1次现况调查,共调查3次,调查内容涉及住培学员的基本情况、对科室带教情况评价、住培学员满意度现状,应用调查反馈机制评价并改进住培工作,比较分析应用调查反馈机制后3次调查结果的差异。结果3次调查问卷的有效回收率分别为:96.2%、95.2%、96.5%。调查项目中带教老师指导学员登记手册、修改学员书写的大病历、认真带教占比分别从2016年6月的73.9%、76.1%、82.7%提高到2017年6月的91.5%、90.1%、95.5%;病例讨论、小讲座、教学查房等带教活动频次≥2次/月的占比分别从2016年6月的70.8%、69.9%、73%提高到2017年6月的90.6%、92.4%、94.6%;完成日常综合测评、出科考核占比从2016年6月的65.0%、82.3%提高到2017年6月的89.2%、95.5%;培训基地的日常管理、激励措施、收入水平、技能培训、专题讲座满意度占比从2016年6月的82.3%、69.9%、79.6%、64.2%、65.9%提高到2017年6月的93.7%、89.7%、91.9%、91.5%、90.6%。结论构建通畅、有效的调查反馈机制,有助于住培工作的持续改进。

Objective This article aimed to explore the effect of investigation feedback mechanism in standardized residency training.Methods 258 residents from 2014 to 2016 residency training programs were selected as research objects.From June 2016 to June 2017,the current situations of the residents were investigated every six months,the investigations were conducted for a total of three times.The contents of the investigation involved basic situations of the residents,evaluation of department teaching and their satisfaction with residency training.Results Three recovery rates of three surveys are 96.2%,95.2%,96.5%,respectively.The rates of teaching faculty guiding residents for handbook registration,modifying residents’writing of medical records and actively engaging in teaching activities raised from 73.9%,76.1%and 82.7%in June 2016 to 91.5%,90.1%and 95.5%in June 2017.The rates of teaching activities like case discussions,lectures,teaching ward rounds with the frequency of 2 times/month or more increased from 70.8%,69.9%and 73%in June 2016,to 90.6%,92.4%and 94.6%in June 2017.The completion rates of daily general assessment and after-department exams arised from 82.3%,and 65.0%in June 2016 to 89.2%and 95.5%in June 2017.The rates of training base daily management,incentive measures,income levels,skills training and seminar satisfactions raised from 82.3%,69.9%,79.6%,64.2%and 65.9%in June 2016 to 93.7%,89.7%,91.9%,91.5%and 90.6%in June 2017.Conclusion The establishment of an unobstructed and effective survey feedback mechanism is helpful to the continuous improvement of residency training.
