摘要
【目的】探讨急性胰腺炎(AP)患者治疗前后血可溶性细胞间黏附分子-1(sICAM-1)、P-选择素及D-二聚体(D-D)水平的变化及其相关性。【方法】选取轻症胰腺炎(MAP)患者36例、重症胰腺炎(SAP)44例及健康体检者(对照组)25例,在入院d1和d7,采集静脉血进行sICAM-1、P-选择素及D-D 检测,分析三组各指标的变化及相关性。【结果】入院d1,sICAM-1、P-选择素和D-D水平为:SAP组>MAP组>对照组(均P<0.05);在入院d7,MAP组和SAP组患者sICAM-1和P-选择素水平较d1明显降低(P<0.05),而D-D水平呈上升趋势(P<0.05);MAP组患者sICAM-1和P-选择素水平低于SAP组(P<0.05),而D-D水平呈相反趋势(P<0.05)。各指标相关性分析显示:在入院d1和d7,AP患者sICAM与P-选择素、D-D呈正相关性(均P<0.01),P-选择素与D-D亦呈正相关性(P<0.01)。【结论】sICAM-1、P-选择素和D-D水平的变化与AP的发生、发展过程存在密切关系,可作为AP患者病情评估的生物学指标。
【Objective】To investigate the changes of serum sICAM-1,P-selectin and D-dimer(D-D) levels in patients with acute pancreatitis and their relationship with the severity of the disease.【Methods】Patients with mild pancreatitis (MAP,n=36) and severe pancreatitis (SAP,n=44) plus 25 healthy people (control group) were selected for the study.On day 1 (d1) and day 7 (d7) of hospital admission,venous blood was collected to detect sICAM-1,P-selectin and D-D .The changes and correlations of each index in the three groups were analyzed.【Results】On the first day of admission,the levels of sICAM-1,P-selectin and D-D in all subjects were SAP group >MAP group>control group (P<0.05).On d7,the levels of sICAM-1 and P-selectin in the MAP group and SAP group were significantly lower than those on d1 (P<0.05),while the D-D level was higher than that of d1 (P<0.05).Furthermore,the levels of P-selectin and sICAM-1 in the MAP group were significantly lower than those in the SAP group (P<0.05),while the level of D-D were the opposite (P<0.05).Pearson or Spearman correlation analysis showed that sICAM was positively correlated with P-selectin and D-D (P<0.01) and P-selectin was positively correlated with D-D (P<0.01) on d1 and d7.【Conclusion】The changes of sICAM-1,P-selectin and D-D are closely related to the occurrence and development of acute pancreatitis,which can be used as biological indicators for evaluating the condition of patients with acute pancreatitis.
