摘要
【目的】探讨丹参川芎嗪联合丁苯酞注射液治疗急性缺血性脑卒中患者的疗效及对血管紧张素Ⅱ(AngⅡ)、脂肪因子趋化素(Chemerin)的影响。【方法】回顾性分析本院收治的302例急性缺血性脑卒中患者临床资,根据治疗方法不同将其分为观察组(丹参川芎嗪+丁苯酞)和对照组(丹参川芎嗪),每组151例。持续治疗2周,NIHSS评分观察患者的神经功能,酶联免疫吸附法检测患者脑钠肽(BNP)、血管紧张素Ⅱ(AngⅡ)、丙二醛(MDA)、超氧化物歧化酶(SOD)、Chemerin水平。【结果】两组血清BNP、AngⅡ、Chemerin、BNP、MDA水平较治疗前降低,SOD水平高于治疗前,且两组间比较差异均有统计学意义(均P<0.05)。治疗后两组NIHSS评分均低于治疗前,且两组比较差异均有统计学意义(P<0.05)。两组均随访2个月,观察组预后不良结局发生率为3.31%(5/151)低于对照组的9.93%(15/151),差异有统计学意义(χ^2=5.355,P=0.021<0.05)。【结论】丹参川芎嗪注射液联合丁苯酞注射液可明显改善急性缺血性脑卒中患者的神经功能,且预后好,这可能与降低患者BNP、MDA、Chemerin水平,提高机体抗氧化能力有关。
【Objective】To investigate the effect of salvia miltiorrhiza ligustrazine and butylphthalide injection on angiotensin Ⅱ(Ang Ⅱ),brain natriuretic peptide (BNP) and adipokinin factor (chemerin) in patients with acute ischemic stroke.【Methods】A total of 302 patients with acute ischemic stroke were randomly divided into the control group (treated with salvia miltiorrhiza ligustrazine injection,n=151) and the observation group (salvia miltiorrhiza ligustrazine + butylphthalide injection,n=151).After 2 weeks of continuous treatment,the NIHSS score was used to observe the neurological function of the patients.The serum levels of BNP,Ang Ⅱ,MDA (malondialdehyde),SOD (superoxide dismutase) and Chemerin were detected by enzyme linked immunosorbent assay.【Results】 The levels of serum BNP,Ang Ⅱ,Chemerin,BNP and MDA in the two groups were lower than those before treatment,and the SOD level was higher than that before treatment.The difference between the two groups was statistically significant (P<0.05).The NIHSS scores of the two groups were lower than those before treatment,and the difference between the two groups was statistically significant (P<0.05).The two groups were followed up with for 2 months.The incidence of poor prognosis in the observation group was 3.31%(5/151),which was lower than 9.93%(15/151) in the control group.The difference was statistically significant (χ^2=5.355,P=0.021<0.05).【Conclusion】The salvia miltiorrhiza ligustrazine combined with butylphthalide injection can obviously improve the neurological function of the patients with acute ischemic stroke,and the prognosis is better.It may be related to reducing the level of plasma BNP and MDA,Chemerin,alleviating the inflammatory reaction and improving the body's antioxidant capacity.
