贝伐珠单抗联合顺铂腹腔灌注治疗胃癌恶性腹腔积液

Intraperitoneal Injection of Bevacizumab plus Cisplatin in the Treatment of Gastric Cancer Complicated with Malignant Ascites

目的观察贝伐珠单抗联合顺铂腹腔灌注治疗胃癌恶性腹腔积液的疗效和安全性。方法选取合并恶性腹腔积液的晚期胃癌患者42例为研究对象,随机分为研究组和对照组,2组均使用顺铂腹腔灌注,研究组在此基础上加用贝伐珠单抗腹腔灌注,比较观察2组腹腔积液控制率、生活质量改善率、治疗后血清肿瘤标志物改变和不良反应。结果研究组腹腔积液控制有效率72.7%,显著高于对照组的45.0%(P=0.032);研究组生活质量改善率显著高于对照组(P=0.031);治疗后,研究组血清癌胚抗原显著高低于对照组(P=0.045);2组不良反应大多为轻度,且2组各不良反应发生率比较差异均无统计学意义(P>0.05)。结论贝伐珠单抗联合顺铂腹腔灌注治疗晚期胃癌恶性腹腔积液疗效确切,且安全性高。

Objective To observe the effect and safety of intraperitoneal injection of bevacizumab plus cisplatin in the treatment of gastric cancer complicated with malignant ascites. Methods A total of 42 patients with advanced gastric cancer complicated with malignant ascites were selected and divided into the study group( n =22)and the control group( n =20). The 20 patients in the control group were treated with intraperitoneal injection of cisplatin alone, and the 22 patients in the study group were treated with intraperitoneal injection of bevacizumab plus cisplatin. The therapeutic effect, quality of life, serum tumor markers and adverse events of the two groups were compared. Results The effective control rate of malignant ascites in the study group was higher than that in the control group(72.7% and 45.5%, P =0.032). The improvement rate of quality of life in the study group was better than that in the control group( P =0.031). After treatment,serum carcinoembryonic antigen level in the study group was lower than that in the control group( P =0.045). Most adverse events were tolerable, and there were no statistical differences in the incidences of adverse events between the two groups( P >0.05). Conclusion Intraperitoneal injection of bevacizumab plus cisplatin is superior to intraperitoneal injection of cisplatin alone in the treatment of gastric cancer complicated with malignant ascites, and it’s safe too.
