定量CT纹理分析鉴别体内泌尿系草酸盐与非草酸盐结石

Quantitative CT texture analysis in differential diagnosis between calcium oxalate and non-calcium oxalate urinary stones

目的:探讨应用CT纹理分析鉴别泌尿系草酸盐与非草酸盐结石的可行性。方法:对照结石成分鉴定结果,回顾性分析30例草酸盐结石与30例非草酸盐结石患者的CT图像。应用TexRAD软件分析CT图像的纹理特征并比较两种成分结石之间各纹理参数间的差异。结果:CT图像上,草酸盐结石与非草酸盐结石的平均灰度值(Mean)在精细、中等及粗糙纹理图像上(SSF 0~6)差异均有统计学意义(P<0.05),标准差(SD)在精细、中等及粗糙纹理图像上(SSF 0~6)差异均有统计学意义(P<0.05),熵(Entropy)在精细、中等及粗糙纹理图像上(SSF 0~6)差异均无统计学意义(P>0.05),正性像素平均值(MPP)在精细、中等及粗糙纹理图像上(SSF 0~6)差异均有统计学意义(P<0.05),偏度(Skewness)在中等纹理图像上(SSF 3~5)差异有统计学意义(P<0.05),峰度(Kurtosis)在无滤过及精细纹理图像上(SSF 0~2)差异有统计学意义(P<0.05)。结论:草酸盐与非草酸盐结石的CT纹理分析参数有一定差异,CT纹理分析具有鉴别两类结石的潜在临床应用价值。

Objective: To investigate the feasibility of using CT texture analysis in differentiating calcium oxalate stones and non-calcium oxalate stones. Methods: CT images of 30 patients with calcium oxalate stones and 30 patients with non-calcium oxalate stones were analyzed retrospectively.TexRAD software was used to analyze the textural features of CT images and compared the differences in each texture parameter between calcium oxalate stones and non-calcium oxalate stones. Results: On CT images,the mean gray value (mean) of oxalate and non-oxalate stones were significantly different ( P < 0.05) in the medium and coarse texture image (SSF 3~6).SD in rough texture image (SSF 6) has significant difference ( P <0.05).Entropy in fine,medium,and coarse texture image (SSF 0~6) had no significant difference ( P >0.05).The difference of MPP in medium and coarse texture image (SSF 5~6) was statistically significant ( P <0.05).Skewness was significantly different in unfiltered and medium texture images (SSF 0,4)( P <0.05),and Kurtosis was significantly different in unfiltered and fine texture images (SSF 0~2)( P <0.05). Conclusion: CT texture analysis has potential clinical application values in differentiating calcium oxalate stones and non-calcium oxalate stones.
