摘要
目的:探讨血清 miR- 210和 miR- 155水平对先兆子痫(PE)早期诊断的临床价值。方法:选取PE孕妇(PE组)和健康孕妇(对照组)各60例,收集所有孕妇的临床资料(年龄、孕周、身体质量指数、收缩压、舒张压、血红蛋白、尿素氮和肌酐等),采用实时定量荧光(PCR)法检测孕妇血清 miR -210和 miR -155的相对表达水平,采用接受者操作特性曲线( ROC )分析候选 mi RNA作为PE潜在诊断标志物的准确性。结果:两组孕妇的年龄、身体质量指数(BMI)、孕周、血红蛋白和尿素氮差异无统计学意义( P >0.05),PE组孕妇收缩压、舒张压和肌酐水平均明显高于对照组孕妇,差异有高度统计学意义( P <0.01);PE组孕妇血清 miR -210和 miR -155水平均高于对照组孕妇,差异有统计学意义( P <0.05);孕妇血清 miR -210的 ROC 曲线下面积为0.904(95% CI为 0.847~0.961, P <0.01), miR -155的 ROC 曲线下面积为0.843(95% CI 为0.766~0.920, P <0.01),孕妇血清 miR -210水平用于预测PE的灵敏度和特异性均高于 miR -155。结论:孕妇血清 miR -210和 miR -155水平可为PE的早期诊断提供参考, miR -210的诊断价值高于 miR -155。
Objective: To investigate the clinical value of serum miR-210 and miR-155 levels as early diagnostic markers of pre-eclampsia (PE). Methods: Sixty pregnant women with PE (PE group) and sixty healthy pregnant women (control group) were enrolled. The clinical data of all pregnant women ( age, gestational age, body mass index (BMI), systolic blood pressure, diastolic blood pressure, hemoglobin, urea nitrogen and creatinine) were collected. Real-time quantitative PCR (qPCR) was used to detect the relative expression levels of maternal serum miR-210 and miR-155, and the receiver operating characteristic curve ( ROC ) was used to analyze the accuracy of candidate miRNAs as potential diagnostic markers for PE. Results: There was no significant difference in age, BMI, gestational age, hemoglobin and urea nitrogen between the two groups ( P >0.05). The systolic blood pressure, diastolic blood pressure and creatinine level in the PE group were significantly higher than those in the control group ( P <0.01). The serum levels of miR-210 and miR-155 in the PE group were higher than those in the control group ( P <0.05). The area under the ROC curve of serum miR-210 in pregnant women was 0.904 (95% CI 0.847~0.961, P <0.01), and that of miR-155 was 0.843 (95% CI 0.766~0.920, P <0.01). The sensitivity and specificity of The serum miR-210 level to predict PE are higher than that of miR-155 in pregnant women. Conclusion: The serum levels of miR-210 and miR-155 in pregnant women can be biomarkers for early diagnosis of PE. The diagnostic value of miR-210 is higher than that of miR-155.
