双源CT双能量虚拟平扫技术在泌尿系结石CTU检查中的应用效果评价 被引量:2

Evaluation of Application Effect of Dual-source CT Dual-energy Virtual Flat-scan Technique in CTU Examination of Urinary Calculi

摘要 目的分析双源CT双能量虚拟平扫技术(VNCT)在泌尿系结石CTU检查中的应用效果。方法回顾性分析该院在2016年11月-2018年12月期间收治的135例泌尿结石患者的病历资料,所有患者均进行双源CT检查,检查序列有常规平扫、增强扫描与排泄期双能量扫描,并获取线性融合平扫图像(DNCT)、VNCT图像。比较TNCT、DNCT、VNCT 3种图像的肾实质CT值、SNR和结石检出数量。结果 3组图像中,正常肾实质平均CT值最大的为VNCT图像(44.23±2.54)%,其次DNCT图像(32.78±2.42)%,最小的为TNCT图像(32.32±2.01)%;SNR由大到小分别为VNCT(3.68±0.32)%、TNCT(2.42±1.03)%、DNCT(2.50±0.26)%;其中VNAT图像的肾实质CT值和SNR与DNCT、VNCT比较差异有统计学意义(P<0.05);3组图像质量住院评分在3~5分之间,平均得分(3.5±0.4)分,所有图像评分均超过3分,满足了影像学诊断的要求;3组图像对结石的检出率无明显差异,且对直径超过2 mm的结石检出率均达到了100.00%。结论双源CT双能量虚拟平扫技术在泌尿系结石CTU检查具有较高的临床应用价值,其对于结石的检出率较高,图像质量好,并且对患者辐射较小,应用安全可靠。 Objective To analyze the application effect of dual-source CT dual-energy virtual plain scanning technique (VNCT) in CTU investigation of urinary calculi.Methods A retrospective analysis of the medical records of 135 patients with urinary calculi admitted to our hospital from November 2016 to December 2018.All patients underwent dual-source CT examination.The examination sequence included routine plain scan,enhanced scan and excretion period.Dual energy scanning and acquisition of linear fused flat image (DNCT),VNCT images,the CT values,SNR and number of stones detected by TNCT,DNCT and VNCT were compared.Results Among the three groups of images,the average CT value of normal renal parenchyma was VNCT image (44.23±2.54)%,followed by DNCT image (32.78±2.42)%,and the smallest was TNCT image (32.32±2.01)%;SNR from large to small were VNCT (3.68±0.32)%,TNCT (2.42±1.03)%,and DNCT (2.50±0.26)%.The CT values and SNR of renal parenchyma in VNAT images were statistically significant compared with DNCT and VNCT (P<0.05).The image quality hospitalization score was between 3 and 5 points,the average score was (3.5±0.4)points,and all the image scores exceeded three points,which satisfied the requirements of imaging diagnosis.The detection rate of stones in the three groups of images was not obvious.Differences,and the detection rate of stones exceeding 2 mm in diameter has reached 100.00%.Conclusion Dual-source CT dual-energy virtual plain scanning technique has high clinical application value in urinary calculi CTU examination.It has higher detection rate of stone,good image quality,less radiation to patients,and safe and reliable application.
