摘要
目的评价超声心动图与N末端脑利钠肽前体在川崎病诊断中的应用价值。方法方便选取该院2016年1月-2018年12月收治的46例川崎病患儿作为观察组,选取同期46例肺炎支原体感染或病毒感染引起的发热、皮疹患儿为对照组,分别行超声心动图及N末端脑利钠前体肽检测;比较两组各参数指标差异性。结果观察组NTproBNP表达水平(1 152.47±214.36)ng/L;对照组NT-proBNP表达水平(54.62±8.43)ng/L;差异有统计学意义(t=26.693,P=0.000);左冠状动脉及右冠状动脉内径均高于对照组(2.41±0.43)mm vs (2.18±0.24)mm;(2.18±0.16)mm vs (1.76±0.15)mm,差异有统计学意义(P<0.05);急性期冠状动脉内径扩张率10.87%低于恢复期的41.30%,差异有统计学意义(χ^2=9.527,P=0.002);急性期N末端脑利钠肽前体表达水平、左冠状动脉及右冠状动脉内径低于恢复期,差异有统计学意义(P<0.05)。经线性Logistic回归分析,急性期N末端脑利钠肽前体表达水平与恢复期左冠状动脉及右冠状动脉内径呈正相关性(B=-5.834,Wald=6.247,r=0.742,P=0.008);但与急性期左冠状动脉及右冠状动脉内径无相关性(B=-0.215,Wald=1.247,r=0.12,P=0.158)。结论在川崎病诊断中应用超声心动图与N末端脑利钠肽前体诊断,具较高的诊断价值。
Objective To evaluate the value of echocardiography and N-terminal pro-brain natriuretic peptides in the diagnosis of Kawasaki disease.Methods A total of 46 children with Kawasaki disease who were admitted to our hospital from Janary 2016 to December 2018 were convenient selected as observation group.46 children with fever and rash caused by Mycoplasma pneumoniae infection or viral infection were selected as the control group.Echocardiography and echocardiography were performed respectively.N-terminal brain natriuretic peptide detection;comparison of the differences between the two groups of parameters.Results The expression level of NT-proBNP was observed in the observation group (1 152.47±214.36)ng/L;the expression level of NT-proBNP in the control group was (54.62±8.43)ng/L;statistically significant (t=26.693,P=0.000);left coronary artery diameter of the right coronary artery was higher than that of the control group (2.41±0.43)mm vs (2.18±0.24)mm;(2.18±0.16)mm vs (1.76±0.15)mm,which was statistically significant (P<0.05).The acute coronary artery diameter expansion rate was 10.87% lower than the recovery period of 41.30% was statistically significant (χ^2=9.527,P=0.002);the expression level of N-terminal brain natriuretic peptide precursor,left coronary artery and right coronary artery in the acute phase was lower than the recovery period,which was statistically significant (P<0.05).Linear logistic regression analysis showed that the expression of N-terminal pro-brain natriuretic peptide in the acute phase was positively correlated with the diameter of the left coronary artery and right coronary artery during the recovery period (B=-5.834,Wald=6.247,r=0.742,P=0.008).However,there was no correlation between the left coronary artery and the right coronary artery diameter (B=-0.215,Wald=1.247,r=0.12,P=0.158).Conclusion The application of echocardiography and N-terminal pro-brain natriuretic peptide diagnosis in the diagnosis of Kawasaki disease has a high diagnostic value.
