超声引导下经皮肝穿刺胆管造瘘电子胆道镜取石术对肝内胆管结石患者并发症的影响分析

Analysis on effects of ultrasound-guided percutaneous transhepatic cholangiography electronic choledochoscopy lithotomy in treatment of intrahepatic cholelithiasis

目的评估超声引导下经皮肝穿刺胆管造瘘电子胆道镜取石术实施在肝内胆管结石患者中对其并发症的影响效果。方法回顾性分析对本医院予以手术治疗的68例肝内胆管结石患者开展临床研究,选择时间是2015年1月~2018年12月,每组34例,试验组开展超声引导下经皮肝穿刺胆管造瘘电子胆道镜取石术治疗,对照组开展腹腔镜下肝部分切除手术治疗,对比两组手术切口长度、手术用时、术中出血量、肝内胆管结石清除总计率、术后并发症总计率。结果试验组手术切口长度、手术用时、术中出血量较对照组减少,差异有统计学意义(P <0.05);试验组肝内胆管结石清除总计率较对照组提升,差异有统计学意义(P <0.05);试验组术后并发症总计率较对照组减少,差异有统计学意义(P <0.05)。结论对肝内胆管结石患者实行超声引导下经皮肝穿刺胆管造瘘电子胆道镜取石术治疗获得良好效果,可进一步推广使用。

Objective To evaluate the effects of ultrasound-guided percutaneous transhepatic cholangiography electronic choledochoscopy lithotomy on complications of patients with intrahepatic cholelithiasis. Methods 68 patients with intrahepatic cholelithiasis who were given surgical treatment in our hospital from January 2015 to December 2018 were retrospectively analyzed and clinical studied. There were 34 cases in each group. The patients in the experimental group were treated with ultrasound-guided percutaneous transhepatic cholangiography electronic choledochoscopy lithotomy while patients in the control group were treated with laparoscopic partial hepatectomy. The length of the incision, the operative time, intraoperative blood loss, the total removal rate of intrahepatic bile duct stones and the total postoperative complication rate were compared between two groups. Results The length of the incision, the operative time and intraoperative blood loss of the experimental group were less than those of the control group. The difference was statistically significant(P < 0.05). The total removal rate of intrahepatic bile duct stones of the experimental group was higher than that of the control group. The difference was statistically significant(P < 0.05). The total postoperative complication rate of the experimental group was lower than that of the control group. The difference was statistically significant(P < 0.05). Conclusion Ultrasound-guided percutaneous transhepatic cholangiography electronic choledochoscopy lithotomy in treatment of patients with intrahepatic cholelithiasis can achieve good curative effects, which can be further promoted and applied.
