内镜下经鼻型肠梗阻导管在外科手术后肠梗阻患者中的应用

Application of endoscopic transnasal intestinal obstruction catheter in patients with postoperative intestinal obstruction

目的 探讨内镜下经鼻型肠梗阻导管在缓解术后肠梗阻中的应用价值。方法回顾性分析2017年1月至2018年3月我院收治的52例外科手术后发生粘连性肠梗阻患者的临床资料。其中26例经内镜安置肠梗阻导管治疗(观察组),另26例置鼻胃管行胃肠减压治疗(对照组)。分别对比2组患者的置管后腹部症状缓解时间、排气解便时间、CT提示梗阻缓解时间、置管时间及住院时间等,观察其临床疗效。结果观察组患者在置管24h后腹部症状缓解、48h后排气排便情况均优于对照组,梗阻缓解时间、平均置管天数及住院时间较对照组明显缩短,2组比较差异有统计学意义(P<0.05)。结论内镜下经鼻型肠梗阻导管能有效地缓解外科术后所致肠梗阻,并预防粘连性肠梗阻的复发。

Objective To investigate the application value of transnasal intestinal obstruction catheter under endoscope in relieving postoperative intestinal obstruction.Methods A retrospective analysis was performed on 52 cases of postoperative intestinal obstruction who were admitted into our hospital from January 2017 to March 2018.Among them,26 cases were treated with intestinal obstruction catheter through endoscope(the observation group),and the other 26 cases were treated with nasogastric catheter for gastrointestinal decompression(the control group).The relief time of abdominal symptoms after catheterization,the time of defecation,the relief time of obstruction indicated by CT,the time of intubation and the time of hospitalization were respectively compared in the 2 groups to observe the clinical efficacy.Results In the observation group,the status of abdominal symptoms after 24 hours and exhaust and defecation condition after 48 hours were better than those in the control group.The time of obstruction remission,average days of catheterization and length of hospital stay were significantly shorter than those in the control group,with statistically significant differences(P<0.05).Conclusion Transnasal intestinal obstruction catheter can effectively relieve postoperative intestinal obstruction and prevent recurrence of adhesive intestinal obstruction.
