B7-H3基因在食管鳞癌患者外周血中的表达及其临床意义

Expression and clinical significance of B7-H3 gene in peripheral blood of patients with esophageal squamous cell carcinoma

目的 探讨B7-H3基因及其可溶性分子在食管鳞癌患者外周血中的表达,分析其与食管鳞癌临床指标及预后的关系。方法选取2010年10月至2012年10月河北医科大学第四医院接受手术治疗并保留了患者信息和外周血标本的食管鳞癌患者52例(食管鳞癌组)和30名体检健康者(对照组)为研究对象。实时定量PCR法检测外周血中B7-H3mRNA表达水平,ELISA法检测血中可溶性sB7-H3分子的表达水平。结果B7-H3mRNA在对照组的相对表达量为2.01±0.12,在食管鳞癌组相对表达量为5.83±0.47,差异有统计学意义(P<0.05);食管鳞癌组外周血sB7-H3的平均水平为(42.19±12.15)ng/ml,对照组外周血sB7-H3的平均水平为(12.67±7.19)ng/ml,差异具有统计学意义(P<0.05);B7-H3mRNA表达与肿瘤TNM分期及复发有关(P<0.05);B7-H3基因高表达患者生存时间较B7-H3基因低表达患者短(P<0.05)。结论B7-H3基因在食管鳞癌患者外周血中高表达,可作为食管鳞癌的参考指标,且对食管鳞癌进展及复发有一定的预测作用。

Objective To investigate the expression of B7-H3 gene in peripheral blood of esophageal squamous cell carcinoma (ESCC) and its relationship with clinicopathologic features, including postoperative recurrence and survival status.Methods Fifty-two cases of ESCC who received surgery and30 healthy controls were enrolled in this cohort where 5ml anticoagulant blood collected from October2010 to October2012.Quantitative PCR was used to examine the expression of B7-H3 mRNA.ELISA was used to detect the level of sB7-H3 in the plasma of the patients with esophageal cancer and healthy controls.Results The comparative B7-H3 mRNA expression was 1.09±0.12 in healthy controls, and 5.83±0.47 in ESCC patients peripheral blood.The expression of B7-H3 was significantly associated with clinical stage and patients ’ recurrence ( P < 0.05).The average level of sB7-H3 was only(12.67±7.19)ng/ml in healthy controls, which was significantly increased in ESCC patients (42.19 ±12.15) ng/ml.The survival time of patients with high expression of B7-H3 gene was significantly shorter than that of patients with low expression of B7-H3 gene.Conclusions B7 -H3 gene was abnormally overexpressed in the peripheral blood of ESCC patients, which has potential implications for the development and recurrence of patients in clinical treatment.
