Y型负压吸引鞘在女性输尿管中下段结石治疗中的应用效果

Application effect of Y type negative pressure suction sheath in the treatment of female middle and lower ureteral calculi

目的研究Y型负压吸引鞘在女性输尿管中下段结石治疗中的有效性与安全性。方法选取2016年1月~2018年11月南康友好医院收治的79例女性输尿管中下段结石患者作为研究对象,随机分为观察组(41例)与对照组(38例)。观察组采用输尿管硬镜进入结石下方,置入16FY型负压吸引鞘,接负压吸引器,钬激光碎石;对照组直接输尿管硬镜进入输尿管,找到结石,用钬激光将结石击碎,比较两组患者的临床效果。结果观察组患者的治愈率为97.56%,高于对照组的76.32%,差异有统计学意义(P<0.05);观察组患者的手术时间、住院时间均短于对照组,差异有统计学意义(P<0.05);观察组患者的出血量少于对照组,差异有统计学意义(P<0.05);观察组患者的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论女性输尿管中下段结石采用Y型负压吸引鞘钬激光碎石取石,具有高效、安全、实用、副损伤小及并发症少等优点,值得在临床推广应用。

Objective To study the efficacy and safety of Y type negative pressure suction sheath in the treatment of female middle and lower ureteral calculi. Methods A total of 79 female patients with middle and lower ureteral calculi admitted to Nankang Friendship Hospital from January 2016 to November 2018 were selected as the subjects. They were randomly divided into observation group (41 cases) and control group (38 cases). In the observation group, ureteroscopy was used to enter the lower part of the calculi, and a 16 F Y negative pressure suction sheath was placed, followed by a negative pressure aspirator, holmium laser lithotripsy. In the control group, ureteroscope was directly inserted into the ureter to find the calculi. Holmium laser was used to break the calculi. The clinical effects of the two groups were compared. Results The cure rate of the observation group was 97.56%, which was higher than that of the control group (76.32%), and the difference was statistically significant (P<0.05). The operation time and hospital stay time of the observation group were shorter than those of the control group, and the differences were statistically significant (P<0.05). The amount of blood loss in the observation group was less than that in the control group, and the difference was statistically significant (P<0.05). The total incidence rate of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). Conclusion The Y type negative pressure suction holmium sheath laser lithotripsy for female middle and lower ureteral calculi has the advantages of high efficiency, safety, practicality, small collateral damage and few complications, and is worthy of clinical application.
