关节镜下有限清理术治疗膝关节骨性关节炎的临床效果

Clinical effect of limited debridement under arthroscope in the treatment of knee osteoarthritis

目的评价关节镜下有限清理术治疗膝关节骨性关节炎的临床效果。方法选取2017年11月~2018年10月我院收治的50例膝关节骨性关节炎患者作为研究对象,按照患者入院后病床尾号单双号不同分为对照组与观察组,每组各25例。对照组开展广泛清理术,观察组开展关节镜下有限清理术,比较两组患者的手术时间、术后并发症发生情况、1个月与3个月膝关节Lysholm评分、膝关节功能优良率。结果观察组患者的手术时间短于对照组,差异有统计学意义(P<0.05);两组患者的术后并发症总发生率比较,差异无统计学意义(P>0.05);两组患者术前膝关节Lysholm评分比较,差异无统计学意义(P>0.05);观察组患者术后1、3个月的膝关节Lysholm评分均明显高于对照组,差异有统计学意义(P<0.05);观察组患者的膝关节功能优良率为88.00%,明显高于对照组的48.00%,差异有统计学意义(P<0.05)。结论关节镜下有限清理术治疗膝关节骨性关节炎具有效果显著、创伤小、恢复快的特点,值得临床广泛应用。

Objective To evaluate the clinical effect of limited debridement under arthroscope in the treatment of knee osteoarthritis. Methods From November 2017 to October 2018, 50 patients with knee osteoarthritis admitted to our hospital were selected as the study objects. According to the patients′ even or odd bed numbers, they were divided into the control group and the observation group, with 25 cases in each group. The control group were treated with extensive cleaning technique,while the observation group carried out limited debridement under arthroscopy, the operative time, postoperative complications, after 1 and 3 month Lysholm knee scores, the rate of good function of knee joint were compared. Results The operation time of the observation group was shorter than that of the control group, the difference was statistically significant (P<0.05). The total incidence of postoperative complication between the two groups had no statistically significant difference (P>0.05). There was no statistically significant difference in preoperative Lysholm score of the knee between the two groups before treatment (P>0.05). The Lysholm scores of the knee in the observation group after 1 and 3 months were significantly higher than those in the control group, the differences were statistically significant (P<0.05). The excellent and good rate of knee joint function in the observation group was 88.00%, significantly higher than 48.00% in the control group, the difference was statistically significant (P<0.05). Conclusion Limited debridement under arthroscope in the treatment of knee osteoarthritis has the characteristics of significant curative effect, small trauma and quick recovery, and is worthy of wide clinical application.
