子宫动脉栓塞术联合药物流产治疗子宫瘢痕妊娠的临床效果

Clinical effect of uterine artery embolization combined with drug abortion in the treatment of uterine scar pregnancy

目的探讨子宫动脉栓塞术联合药物流产治疗子宫瘢痕妊娠的临床效果。方法选取2017年1月~2018年11月我院收治的40例子宫瘢痕妊娠患者作为研究对象,按照分段随机化方法分为观察组(18例)与对照组(22例)。观察组采用子宫动脉栓塞术联合药物流产治疗,对照组采用甲氨蝶呤联合米非司酮治疗。比较两组的手术相关指标、子宫切除率及治疗效果。结果观察组开始清宫术用时及术后阴道出血时间短于对照组,术中出血量及子宫切除率均低于对照组,治疗总有效率高于对照组,差异均有统计学意义(P<0.05)。结论子宫瘢痕妊娠的治疗中应用子宫动脉栓塞术联合药物流产治疗能提高治疗效果,缩短开始清宫术用时及术后阴道流血时间,减少术中出血及子宫切除的风险。

Objective To explore the clinical effect of uterine artery embolization combined with drug abortion in the treatment of uterine scar pregnancy. Methods A total of 40 cases of uterine scar pregnancy in our hospital from January 2017 to November 2018 were selected as the research objects, they were divided into observation group (18 cases) and control group (22 cases) according to the method of sectional randomization. The observation group was treated with uterine artery embolization combined with drug abortion, while the control group was treated with Methotrexate combined with Mifepristone. The operation related indexes, the rate of hysterectomy and the therapeutic effect were compared between the two groups. Results In the observation group, the time of vaginal bleeding was shorter than that in the control group, the amount of bleeding and the rate of hysterectomy were lower than those in the control group, and the total effective rate of treatment was higher than that in the control group, the differences were statistically significant (P<0.05). Conclusion Uterine artery embolization combined with drug abortion in the treatment of uterine scar pregnancy can improve the therapeutic effect, shorten the time of starting uterine curettage and vaginal bleeding after operation, and reduce the risk of intraoperative bleeding and hysterectomy.
