半乳糖凝集素3及微量白蛋白尿与高血压患者左室肥厚的关系研究

Relationship among galectin 3, microalbuminuria and left ventricular hypertrophy in hypertensive patients

目的探讨半乳糖凝集素3(Gal-3)及微量白蛋白尿(MAU)与高血压患者左室肥厚(LVH)的关系。方法选择2016年1月至2017年6月于西安交通大学第二附属医院老年心血管科收治的79例高血压LVH患者作为研究组,选择同期我院收治的80例高血压非LVH患者作为对照组。对两组病例资料进行调查分析,分别检测两组患者血清Gal-3及MAU水平,并应用单因素与多因素回归分析,分析影响高血压患者LVH的因素。结果研究组Gal-3和MAU水平均显著高于对照组(P<0.05)。经Logistic回归分析发现,高血压病程≥5年、高血压控制未达标、Gal-3≥16.27 ng/mL与MAU≥28.75 mg/g是高血压LVH的独立危险因素(P<0.05)。ROC分析结果表明,Gal-3和MAU的AUC分别为0.803和0.825。结论高血压病程≥5年、高血压控制未达标、Gal-3≥16.27 ng/mL与MAU≥28.75 mg/g是高血压LVH的独立危险因素,检测Gal-3、MAU对防治高血压LVH具有一定的临床应用价值。

Objective To discuss the relationship among galectin 3(Gal-3), microalbuminuria(MAU) and left ventricular hypertrophy(LVH) in hypertensive patients. Methods The hypertensive patients with LVH(n=79, study group) and hypertensive patients without LVH(n=80, control group) were chosen from Department of Geriatric Cardiovascular Medicine in Second Affiliated Hospital of Xi’an Jiaotong University from Jan. 2016 to June 2017. The data of disease history was investigated and analyzed, and levels of Gal-3 and MAU were detected in 2 groups. The influencing factors of LVH were analyzed by using single-factor and multi-factor regression analysis in hypertensive patients. Results The levels of Gal-3 and MAU were higher in study group than those in control group(P<0.05). The results of Logistic regression analysis showed that hypertensive course≥5 years, failure of hypertension control, Gal-3≥16.27 ng/ml and MAU≥28.75 mg/g were independent risk factors of LVH in hypertensive patients(P<0.05). The results of ROC analysis showed that AUC of Gal-3 was 0.803 and AUC of MAU was 0.825. Conclusion Hypertensive course≥5 years, failure of hypertension control, Gal-3≥16.27 ng/ml and MAU≥28.75 mg/g are independent risk factors of LVH in hypertensive patients. The detections of Gal-3 and MAU have some clinical application value to the prevention and treatment of LVH.
