补髓生血颗粒对慢性再生障碍性贫血负性造血调控因子的影响 被引量:8

Effect of Busui Shengxue Granule on Negative Hematopoietic Regulating Factor in Chronic Aplastic Anemia

摘要 目的观察补髓生血颗粒治疗慢性再生障碍性贫血的临床疗效及对负性造血调控因子的影响。方法选取2016年9月—2018年1月收治的慢性再生障碍性贫血患者128例,按照随机数字表法分为观察组(63例)和对照组(65例),观察组使用补髓生血颗粒治疗,对照组使用司坦唑醇片治疗,2组患者均治疗6个月。统计2组患者治疗后临床疗效及治疗前后中医症状积分;检测各组治疗前后外周血象的变化和骨髓中TNF-α、IFN-γmRNA表达情况。结果观察组总有效率(76.19%,48/63)明显高于对照组(61.54%,40/65)(P<0.05);治疗后2组的症状积分及外周血象较治疗前明显改善,且观察组较对照组改善更为明显(P<0.05);治疗后2组骨髓TNF-α与IFN-γmRNA表达水平均较治疗前有所降低,观察组较对照组明显下降(P<0.05)。结论补髓生血颗粒通过调节细胞因子TNF-α与IFN-γmRNA表达水平来改善造血微环境,恢复骨髓造血功能,从而改良慢性再生障碍性贫血患者临床症状及外周血象,提高其临床疗效。 Objective To observe the clinical effect of Busui Shengxue granule on chronic aplastic anemia patients and the influence of negative hematopoietic regulatory factors. Methods 128 patients with chronic aplastic anemia admitted to our hospital from September 2016 to January 2018 were selected as the research objects. They were divided into the observation group (n=63) and the control group (n=65) according to random number table method. The observation group was treated with Busui Shengxue granule, while the control group was treated with stanzolol tablets. Both groups were treated for 6 months. The clinical efficacy of the 2 groups after treatment and the scores of TCM symptoms before and after treatment were counted, and the changes of peripheral blood picture and the expression of TNF-α and IFN-γ in bone marrow of each group before and after treatment were detected. Results The total effective rate (76.19%, 48/63) of the observation group was significantly higher than that of the control group (61.54%, 40/65)(P<0.05), the symptom score and peripheral blood picture of the 2 groups significantly improved after treatment, and the improvement of the observation group was more obvious than that of the control group (P<0.05), the expression levels of TNF-α and IFN-γ in bone marrow of the 2 groups increased after treatment, and the observation group was significantly higher than that of the control group (P<0.05). Conclusion Busui Shengxue granule can improve the hematopoietic microenvironment and restore the hematopoietic function of bone marrow by regulating the expression levels of cytokines TNF-α and IFN-γ, so as to improve the clinical symptoms and peripheral blood picture of patients with chronic aplastic anemia and improve its clinical efficacy.
