围术期保温护理措施用于腹腔镜胃肠手术胃肠功能及护理效果观察

Effect of perioperative thermal insulation nursing measures on gastrointestinal function and nursing effect of patients undergoing laparoscopic gastrointestinal surgery

目的 探讨围术期保温护理措施对于腹腔镜胃肠手术患者术后胃肠功能及护理效果的影响。方法 选取本院2017年1月至2018年12月入住本院的行腹腔镜胃肠手术患者86例,运用红篮球法将患者分为观察组(43例)、对照组(43例),观察组采用围术期保温护理措施,对照组采用常规围术期护理,对比两组患者的术后胃肠功能及护理效果。结果 观察组患者术后排气时间(44.79±3.94)h、排便时间(2.92±0.39)d、进食时间(34.16±5.76)h、肠鸣音恢复时间(31.38±2.73)h,均显著低于对照组(均P<0.05);观察组患者术后7d血清DAO水平为(2.08±0.62)U/ml,显著低于对照组(P<0.05),与术前1d(1.85±0.51)U/ml 比较,差异无统计学意义(P > 0.05);观察组患者术中体温为(36.35±0.40)℃、术后体温越来越(36.28±0.42)℃,均高于对照组(均P<0.05);观察组患者各个时间点比较差异均无统计学意义(均P > 0.05)。结论 围术期保温护理措应用于腹腔镜胃肠手术患者,能够促进患者胃肠功能恢复,提高胃肠屏障功能,避免患者发生术中低体温情况,护理效果显著。

Objective To explore the effect of perioperative heat preservation nursing measures on gastrointestinal function and nursing effect of patients undergoing laparoscopic gastrointestinal surgery. Methods 86 patients admitted to our hospital from January 2017 to December 2018 were divided into an observation group (n = 43) and a control group (n = 43). The observation group was treated with perioperative heat preservation, and the control group with routine perioperative nursing. The postoperative gastrointestinal function and nursing effect of the two groups were compared. Results The postoperative exhaust time, the defecation time, the feeding time, and the bowel sound recovery time were (44.79±3.94) h,(2.92±0.39) d,(34.16±5.76) h,(31.38±2.73) h in the observation group, which were significantly lower than those in the control group (all P<0.05). The serum DAO level in the observation group 7 days after the operation was (2.08±0.62) U/ml, which was significantly lower than that in the control group (P < 0.05). Before the operation, the level of DAO was (1.85±0.51) U/ ml in the observation group, with no statistical difference from that after the operation (P> 0.05). The intra- and postoperative body temperatures were (36.28 ±0.42)℃ and (36.35 ±0.40)℃ in the observation group, which were significantly higher than those in the control group (both P < 0.05). There were no statistical differences between the time points in the observation group (P>0.05). Conclusion Perioperative heat preservation nursing measures for patients undergoing laparoscopic gastrointestinal surgery can promote the recovery of gastrointestinal function and gastrointestinal barrier function, and avoid hypothermia during the operation. The nursing effect is remarkable.
