血清LDL-C、FIB、颈动脉斑块与TIA后脑梗死相关分析

摘要 目的探讨血清低密度脂蛋白胆固醇(LDL-C)、纤维蛋白原(FIB)及颈动脉斑块与短暂性缺血发作(TIA)后脑梗死的关系。方法选取2017年6月—2018年12月在广东省普宁市人民医院院治疗的TIA患者201例,根据7d内是否发生脑梗死分为脑梗死组(n=56)和无脑梗死组(n=145),检测LDL-C、FIB和颈动脉斑块,同时评估Essen评分。结果脑梗死组血清LDL-C、FIB及颈动脉斑块比例分别为(5.16±1.03)mmol/L、(3.49±1.06)g/L和92.86%,明显低于无脑梗死组(P<0.05);脑梗死组Essen评分、Essen评分+LDL-C、Essen评分+FIB、Essen评分+颈动脉斑块、Essen评分+LDL-C+FIB+颈动脉斑块分别为(5.32±0.45)分、(5.92±0.38)分、(5.58±0.41)分(5.60±0.38)分和(5.89±0.43)分,明显高于无脑梗死组(P<0.05);Essen评分、Essen评分+LDL-C、Essen评分+FIB、Essen评分+颈动脉斑块、Essen评分+LDL-C+FIB+颈动脉斑块预测TIA后脑梗死的ROC曲线下面积分别为0.890、0.930、0.901、0.761和0.737,P<0.05。结论TIA后脑梗死患者血清LDL-C和FIB水平、颈动脉斑块比例较高,其中LDL-C、FIB在联合Essen评分预测TIA后脑梗死发生的价值较好。 Objective To investigate the relationship between low density lipoprotein cholesterol(LDL-C),fibrinogen(FIB), carotid plaque and cerebral infarction after transient ischemic attack(TIA). Methods 201 TIA patients treated in the hospital from June,2017 to December,2018 were selected,according to whether cerebral infarction occurred within 7 days. The patients were di? vided into cerebral infarction group(n=56)and non-cerebral infarction group(n=145). The LDL-C,FIB and carotid plaque were de? tected,and Essen score was evaluated. Results The LDL-C,FIB and proportion of carotid plaque in cerebral infarction group were (5.16±1.03)mmol/L,(3.49±1.06)g/L and 92.86%,which were significantly lower than those in non-cerebral infarction group(P< 0.05).The Essen score,Essen score+LDL-C,Essen score+FIB,Essen score+carotid plaque,Essen score+LDL-C+FIB+carotid plaque in cerebral infarction group were(5.32±0.45),(5.92±0.38),(5.58±0.41),(5.60±0.38)and(5.89±0.43),which were sig? nificantly higher than those in non-cerebral infarction group(P<0.05). The area under ROC curve of Essen score,Essen score+LDLC, Essen score+FIB,Essen score+carotid plaque,Essen score+LDL-C+FIB+carotid plaque predicting cerebral infarction after TIA were 0.890,0.930,0.901,0.761 and 0.737,P<0.05. Conclusion The level of LDL-C and FIB in serum and the proportion of carotid plaque are higher in patients with cerebral infarction after TIA. The LDL-C and FIB has better value in predicting the occurrence of cerebral infarction after TIA combined with Essen score.
