自拟络藤合剂联合射频热凝靶点消融减压术治疗腰椎间盘突出症患者的临床研究

Clinical Effect of Self-made Luoteng Mixture Combined with Radiofrequency Thermocoagulation Target Ablation and Decompression on Lumbar Intervertebral Disc Herniation

目的观察自拟络藤合剂联合射频热凝靶点消融减压术治疗腰椎间盘突出症(LDH)患者的临床效果,并探讨其对患者血清中转化生长因子-β1(TGF-β1)、前列腺素E2(PGE2)及单核细胞趋化蛋白-1(MCP-1)水平的影响。方法将110例患者随机分为对照组和观察组。对照组给予射频热凝靶点消融减压术,观察组则在此基础上给予自拟络藤合剂治疗。对比两组治疗后腰椎功能的优良率,评价痛感、疼痛对日常生活能力的影响程度及腰椎功能,监测治疗前后血清中TGF-β1、PGE2、MCP-1水平。结果观察组腰椎功能优良率为90.91%,明显高于对照组的72.73%(P<0.05)。治疗后两组患者的腰椎疼痛VAS、ODI评分明显下降,腰椎功能JOA评分则明显上升,血清中TGF-β1、PGE2、MCP-1水平明显下降(P<0.05),治疗14d后的变化程度更明显(P<0.05);组间对比观察组变化程度更明显(P<0.05)。结论自拟络藤合剂联合射频热凝靶点消融减压术利于缓解LDH患者的痛感,提高日常生活能力,改善腰椎功能。

Objective:To observe the clinical effect of Self-made Luoteng Mixture combined with radiofrequency thermocoagulation target ablation and decompression on LDH,and to explore its effect on serum levels of TGF-β1,PGE2 and MCP-1.Methods:110 patients were randomly divided into the control group and the observation group.The control group were treated with radiofrequency thermocoagulant target ablation and decompression, while the observation group were treated with the combination of Self-made Luoteng Mixture on that basis.The excellent rate of lumbar spine function between the two groups after treatment were compared;the pain,the effect of pain on daily living ability and lumbar spine function were evaluated,and the levels of serumTGF-β1,PGE2 and MCP-1 were monitored before and after treatment.Results:The excellent rate of lumbar spine function in the observation group was 90.91%,significantly higher than that in the control group(72.73%)(P < 0.05).After treatment,the VAS and ODI scores of lumbar pain and JOA scores of lumbar spine function in the two groups decreased significantly,while the serum levels of TGF-β1,PGE2 and MCP-1 decreased significantly(P < 0.05).After 14 days of treatment,the degree of change was more obvious(P < 0.05),and that was more obvious in the observation group than in the control group(P < 0.05).Conclusion:Self-made Luoteng Mixture combined with radiofrequency thermocoagulation target ablation and decompression is beneficial to relieve pain,and improve daily living ability and lumbar function in LDH patients.
