百令胶囊预防肺癌患者放射性肺损伤的效果观察 被引量:6

Observation on Effect of Bailing Capsule in Preventing Radiation-induced Lung Injury in Patients with Lung Cancer and Its Potential Mechanism

摘要 目的观察百令胶囊对肺癌患者放射性肺损伤的预防作用及对炎性指标C反应蛋白(CRP)、降钙素原(PCT)水平的影响。方法选取肺癌患者80例,随机分为观察组和对照组,每组各40例,对照组给予常规胸部调强放疗,观察组在调强放疗同时口服百令胶囊,4粒/次,3次/d,比较两组患者放射性肺炎及放射性肺纤维化发生率以及放疗期间CRP及PCT水平变化。结果观察组的放射性肺炎发生率10.0%,对照组为30.0%,差异有统计学意义(χ^2=5.189,P<0.05);观察组放射性肺纤维化发生率17.5%,对照组为35.0%,差异无统计学意义(χ^2=3.164,P>0.05);放疗第3周、放疗第6周两组CRP、PCT水平均高于放疗前(P<0.05);放疗第6周时观察组CRP、PCT水平低于对照组(P<0.05)。结论口服百令胶囊可有效预防急性放射性肺炎的发生,其预防机制可能与减轻放疗期间炎性反应,降低炎性指标CRP、PCT水平有关。 Objective To observe the preventive effect of Bailing Capsule on radiation-induced lung injury in lung cancer patients and its effect on the levels of inflammatory markers C-reactive protein (CRP) and procalcitonin (PCT). Methods 80 patients with lung cancer were randomly divided into observation group and control group,40 cases in each group.The control group was given conventional chest intensity-modulated radiotherapy.The observation group was treated with intensity-modulated radiotherapy and oral Bailing capsules,4 capsules once a time,3 times once a day.The incidence of radiation pneumonitis and radiation-induced pulmonary fibrosis and changes in CRP and PCT levels during radiotherapy were compared between the two groups. Results The incidence of radiation pneumonitis in the observation group(10.0%)was lower than that in the control group (30.0%).The difference was statistically significant (χ^2=5.189,P < 0.05).The incidence of radiation pulmonary fibrosis in the observation group (17.5%)was lower than that in the control group (35.0%).There was no statistical significance (χ^2=3.164,P>0.05). The levels of CRP and PCT at the 3rd week and 6th week of radiotherapy were higher than those before radiotherapy in both groups ( P <0.05).The level of CRP and PCT in the observation group was lower than that in the control group at the 6th week of radiotherapy ( P <0.05). Conclusion Oral Bailing Capsule can effectively prevent the occurrence of acute radiation pneumonitis,and its prevention mechanism may be related to reducing the inflammatory reaction during radiotherapy and reducing the levels of inflammatory markers CRP and PCT.
