摘要
目的探讨舒芬太尼联合右美托咪定在颅脑损伤患者术后镇痛镇静中的应用效果。方法选取2016年2月-2018年4月河南省商丘市第一人民医院96例颅脑损伤手术患者,随机数字表法分为试验组(n=48)、对照组(n=48)。对照组术后予以舒芬太尼持续静脉泵入,试验组术后予以舒芬太尼联合右美托咪定持续静脉泵入。对比两组术后6h、12h、24h的静息痛、活动痛评分、镇静情况评分(Ramsay),统计其ICU住院时间、不良发应发生情况。结果试验组术后6h、12h、24h的静息痛、活动痛评分均较对照组低,差异有统计学意义(P<0.05);试验组术后6h、12h、24h的Ramsay评分较对照组高,差异有统计学意义(P<0.05);试验组ICU住院时间较对照组短,差异有统计学意义(P<0.05);试验组不良反应发生率14.58%较对照组37.50%低,差异有统计学意义(P<0.05)。结论颅脑损伤患者术后应用舒芬太尼联合右美托咪定持续静脉泵入可显著提升镇痛镇静效果,缩短患者ICU住院时间,减少不良反应发生。
Objective To investigate the effect of sufentanil combined with dexmedetomidine on postoperative analgesia and sedation in patients with craniocerebral injury.Methods From February 2016 to April 2018,96 patients undergoing craniocerebral injury surgery in the First People's Hospital of Shangqiu City,Henan Province were selected and randomLy divided into the experimental group (n=48) and the control group (n=48).The control group received continuous intravenous infusion of sufentanil after surgery,while the experimental group received continuous intravenous infusion of sufentanil combined with dexmedetomidine after surgery.The resting pain,activity pain score and sedation score (Ramsay) of the two groups at 6h,12h and 24h after operation were compared,and the ICU stay and adverse events were counted.Results The resting pain and activity pain scores of the experimental group were lower than those of the control group at 6h,12h and 24h after operation,and the differences were statistically significant (P < 0.05).The Ramsay score of the experimental group was higher than that of the control group at 6h,12h and 24h after operation,and the difference was statistically significant (P < 0.05).The hospitalization time in ICU of the experimental group was shorter than that of the control group,and the difference was statistically significant (P < 0.05). The incidence of adverse reactions in the experimental group was 14.58%,lower than that in the control group (37.50%),and the difference was statistically significant (P < 0.05).Conclusion Continuous intravenous infusion of sufentanil combined with dexmedetomidine can significantly improve analgesic and sedative effects,shorten ICU stay time and reduce adverse reactions in patients with craniocerebral injury.
