经阴道实时三维超声子宫输卵管造影用于不孕症诊断的临床价值

Clinical value of transvaginal real-time three-dimensional hysterosalpingo-contrastsonography in the diagnosis of infertility

目的:分析经阴道实时三维超声子宫输卵管造影(3D-HyCo-Sy)用于不孕症诊断的临床价值。方法:选择2017年10月—2018年9月治疗的不孕症患者102例,均接受3D-HyCo-Sy和腹腔镜通染液检查(CLP)。观察3D-HyCo-Sy、CLP检查的输卵管通畅性及疼痛程度,并以CLP结果作为金标准,计算3D-HyCo-Sy诊断准确率、特异度、敏感度。结果:102例患者中输卵管共204条,其中CLP检查输卵管通畅62条,通而不畅53条,阻塞89条;3D-HyCo-Sy检查输卵管通畅56条,通而不畅60条,阻塞88条。以CLP结果作为金标准,3D-HyCo-Sy诊断准确率、特异度、敏感度分别为85.48%、97.89%、94.12%;3D-HyCo-Sy组视觉模拟评分法(VAS)评分低于CLP组,差异有统计学意义(P<0.05)。结论:3D-HyCo-Sy用于不孕症诊断中价值较高,利于提升输卵管通畅性诊断效能,为临床诊治提供可靠信息。

Objective:To analyze the clinical value of transvaginal real-time three-dimensional hysterosalpingo-contrastsonography(3D-HyCo-Sy)in the diagnosis of infertility.Methods:All 102 patients with infertility treated in the hospital from October 2017 to September 2018 were selected,and they were given 3D-HyCo-Sy,laparoscopy and dye test(CLP).The tubal patency and pain degree were examined by 3D-HyCo-Sy and CLP.The results of CLP were taken as the gold standard,the diagnostic accuracy,specificity,sensitivity of 3D-HyCo-Sy were calculated.Results:Among the 204 fallopian tubes in 102 patients,there were 62 unobstructed tubes,53 partially obstructed tubes and 89 completely obstructed tubes;3D-HyCo-Sy examination showed that there were 56 unobstructed tubes,60 partially obstructed tubes and 88 completely obstructed tubes.The results of CLP examination were taken as the gold standard,the diagnostic accuracy,specificity,sensitivity of 3D-HyCo-Sy were 85.48%,97.89%and 94.12%.The VAS score of 3D-HyCo-Sy group was lower than CLP group(P<0.05).Conclusion:3D-HyCo-Sy in the diagnosis of infertility has higher application value,it can increase diagnostic efficiency of tubal patency,which can provide reliable information for clinical diagnosis and treatment.
