个体化康复训练联合下肢可调负重支具训练在股骨粗隆间骨折患者术后康复中的效果 被引量:11

Rehabilitation effect of individualized rehabilitation training combined with adjustable weight-bearing brace training on femoral intertrochanteric fracture after operation

摘要 目的分析个体化康复训练联合下肢可调负重支具训练在股骨粗隆间骨折患者术后康复中的效果的影响。方法选取本院2017年1月至2019年1月收治的152例股骨粗隆间骨折患者为研究对象,采用随机数表法将入选患者分为研究组和对照组,每组各76例。两组患者均行外科手术治疗,术后给予个体化康复训练,研究组患者康复训练期间佩戴下肢可调负重支具。根据患者术后恢复情况、术前和术后肌力、Harris髋关节功能评分等指标变化,评价两组患者术后康复效果。结果研究组患者骨折愈合时间显著短于对照组(P < 0.05),术后感染发生率显著低于对照组(P < 0.05),两组患者住院天数比较差异无统计学意义(P > 0.05)。研究组患者术后4周患肢肌力评分显著高于本组术前(P < 0.05),对照组患者术后3周和术后4周患肢肌力评分均显著低于本组术前和同期研究组(均P < 0.05)。两组患者术后6个月Harris髋关节各维度评分和总分均显著高于本组术前(均P < 0.05),且研究组患者术后6个月Harris髋关节各维度评分和总分、功能优良率均显著高于同期对照组(均P < 0.05)。结论个体化康复训练联合下肢可调负重支具训练能够明显改善股骨粗隆间骨折患者术后肌力,促进术后恢复,提高患肢功能恢复效果,值得推广应用。 Objective To explore the effect of individualized rehabilitation training combined with adjustable weight-bearing brace training of lower limbs on the rehabilitation effect of intertrochanteric fracture of femur after operation. Method 152 patients with intertrochanteric fracture of femur admitted to our hospital from January 2017 to January 2019 were randomly divided into study group and control group, 76 cases in each group. Two groups of patients underwent surgical treatment, and individualized rehabilitation training was given after the operation. The patients in study group were wearing adjustable weight-bearing braces of lower limbs during the rehabilitation training. According to the recovery after operation and the changes of muscle strength and Harris hip function score before and after operation, the rehabilitation effect of the two groups were evaluated. Result The healing time of fracture in study group was significantly shorter than that in control group(P < 0.05), and the incidence of postoperative infection was significantly lower than that in control group(P < 0.05). There was no significant difference in hospitalization days between the two groups(P > 0.05). The muscular strength score of affected limbs in study group was significantly higher than that before operation and 4 weeks after operation(all P < 0.05). The muscular strength score of affected limbs in control group at 3 weeks and 4 weeks after operation were significantly lower than those before operation and study group at the same time(all P < 0.05). 6 months after operation, Harris hip joint dimension scores and total scores of the two groups were significantly higher than those before operation(all P < 0.05), and Harris hip joint dimension score and total score, functional excellence rate of study group were significantly higher than those of control group at the same time(all P < 0.05). Conclusion On the basis of individualized rehabilitation training, combined with adjustable weight-bearing brace training of lower limbs can significantly improve the muscle strength of patients with intertrochanteric fracture of femur after operation, promote the recovery after operation and improve the effect of functional recovery, which is worthy of popularization and application.
