Disruption of Drugs in the Intravenous Drug Distribution Center and Strategies for Improving Drug Quality

目的探究静脉用药调配中心药品破损情况及提升策略。方法记录分析我院于2018年1-12月每季度药品破损情况,将造成破损各环节及原因进行分析,同时将有关改进措施制定出来。结果共配制1 514 833组药品,因各种原因产生1 144组药品浪费,其中破损1 031组(0.68‰)。其中西林瓶占36.03%,成品输液占18.25%,破损率最高的是安瓿瓶装药品,高达45.72%;相较于第一季度的破损药品341组(1.14‰),第二、三、四季度对比差异显著有意义(P<0.05);2018存在较高的第一季度药品破损率,大概是由于春节前领药较多,需备药,所以存在很多药品拆包装,逐渐提升破损比例;药品调配过程操作不当332例(32.20%),运送及装卸过程不当276例(26.78%),药品配送及排药操作不当59例(5.72%),拆零时操作不当341例(33.07%),其他23例(2.23%)。结论针对静脉用药调配中心药品破损情况,采用一定干预措施及提升策略,药品破损情况获得极大程度改善,显著降低,具有临床应用价值。

Objective To explore the damage and improvement strategies of drugs in the intravenous drug distribution center. Methods Record and analyze the damage of the drug in our hospital from January 2018 to December 2018, and analyze the various links and causes of damage, and formulate relevant improvement measures. Results A total of 1 514 833 groups of drugs were prepared. For various reasons, 1 144 groups of drugs were wasted, of which 1 031 groups(0.68‰) were damaged. Among them, the vial accounted for 36.03%, the finished infusion accounted for 18.25%, and the highest damage rate was the ampoule bottled drug, which was as high as 45.72%;compared with the first quarter of the damaged drug 341 group(1.14‰), the second, third and fourth quarter comparison the difference was significant(P < 0.05);in 2018, there was a higher rate of drug breakage in the first quarter, probably because there were more drugs taken before the Spring Festival, and there was a need to prepare medicines. Therefore, there were many drugs unpacking and gradually increasing the damage ratio;332 cases(32.20%) were improperly handled, 276 cases(26.78%) were improperly handled and handled, drug distribution and drug discharge 59 cases(5.72%) were improperly operated, 341 cases(33.07%) were improperly operated when they were removed, and 23 cases(2.23%) were used. Conclusion Aiming at the damage of the drug in the intravenous drug distribution center, certain intervention measures and improvement strategies are adopted, and the damage of the drug is greatly improved, significantly reduced, and has clinical application value.
