门冬胰岛素对妊娠期糖尿病患者血糖及妊娠结局的影响

Effects of Insulin on Blood Sugar and Pregnancy Outcomes in Gestational Diabetes Patients

目的研究早期门冬胰岛素治疗对妊娠期糖尿病患者血糖水平及妊娠结局的影响。方法选取我院2016年11月-2018年11月妊娠期糖尿病患者均分为观察组和对照组,观察组在孕周≤32周时给予门冬胰岛素治疗;对照组,在孕周>32周时注射门冬胰岛素。结果观察组空腹血糖、餐后两小时血糖、糖化血红蛋白明显低于对照组(P<0.05);观察组新生儿不良结局发病率明显低于对照组(P<0.05);两组患者的早产率和剖宫产率比较差异无统计学意义(P>0.05),观察组的妊娠期高血压疾病、产后出血、其他并发症发生率明显低于对照组(P<0.05)。结论在妊娠期糖尿病患者早期使用门冬胰岛素治疗,可以有效的降低孕妇血糖,使其保持稳状态,减少新生儿先天疾病发病率,使孕妇妊娠结局更优。

Objective To study the effect of early aspartate insulin therapy on blood glucose level and pregnancy outcome in patients with gestational diabetes mellitus. Methods Patients with gestational diabetes mellitus in our hospital from November 2016 to November 2018 were divided into two groups. The observation group, Aspartate insulin was administered at the gestational ≤ 32 weeks;The control group, Aspartate insulin was administered at gestational age greater than 32 weeks. Results Fasting blood glucose, blood glucose and glycosylated hemoglobin in the observation group were significantly lower than those in the control group(P < 0.05);The incidence of adverse neonatal outcomes in the observation group was significantly lower than that in the control group(P < 0.05);There was no statistically significant difference in the preterm delivery rate and cesarean section rate between the two groups(P > 0.05). The incidence of hypertension during pregnancy, postpartum hemorrhage and other complications in the observation group was significantly lower than that in the control group(P < 0.05). Conclusion Early use of aspartate insulin in patients with gestational diabetes can effectively reduce the blood glucose of pregnant women, maintain a stable state, reduce the incidence of neonatal congenital diseases, and make the pregnancy outcome better.
