单操作孔与多孔胸腔镜下肺叶切除术的对比研究

Comparative Study of Thoracoscopic Lobectomy With Single Operating Hole and Multi-hole

目的通过与多孔胸腔镜下肺叶切除术的对比研究,探讨单操作孔胸腔镜下肺叶切除术的手术技术及临床应用。方法选取我院2014年1月-2018年11月单操作孔胸腔镜下肺叶切除术137例作为研究组,多孔胸腔镜肺叶切除术113例作为对照组;对手术时间、清扫淋巴结个数、手术出血量、术后带管时间、术后住院时间等进行统计分析。结果全部手术顺利完成,无围手术期死亡及重大并发症发生;两组中手术时间、清扫淋巴结个数、术后住院时间及并发症差别无统计学意义(P>0.05),而在术中出血、术后带管时间方面,研究组均低于对照组,差异有统计学意义(P<0.05)。结论相比较于多孔胸腔镜肺叶切除术,单操作孔胸腔镜下肺叶切除术是一种安全有效、创伤更小的手术方式,随着患者对于微创要求的增加,单操作孔胸腔镜下肺叶切除术将在肺部恶性肿瘤外科中占据更重要的地位。

Objective To explore the surgical technique and clinical application of thoracoscopic lobectomy with single operating hole. Methods 137 cases of thoracoscopic lobectomy with single operating hole in January 2014 to November 2018 were in study group. 113 cases of thoracoscopic lobectomy with multi-hole in the same term were in control group;The operating time, the number of lymph nodes removed, the amount of bleeding, the time of tube-carrying of post-operating and hospitalization were analyzed statistically. Results All of the operations were completed successfully, No perioperative death and severe complications;The difference of operating time,number of lymph nodes removed,hospitalizating and incidence of complications was not significant(P >0.05);and the intraoperative bleeding, postoperative tube-carrying time in study group was significantly lower than the control group(P < 0.05). Conclusion Compared with multihole Thoracoscopic lobectomy, single operating hole Thoracoscopic lobectomy is a safe, efficient and less invasive surgical method for the treatment of pulmonary malignancy. With the increase of the patients’ minimally invasive requirements, the single operating hole Thoracoscopic lobectomy will play a more important role in the surgery of lung cancer.
