血清维生素D与乙肝肝硬化疾病的相关性分析

摘要 目的探讨慢性乙型肝炎肝硬化患者血清维生素D水平与乙型肝炎病毒(hepatitis B virus,HBV)复制及肝脏纤维化程度的相关性。方法收集2016年11月-2018年11月在青岛市第六人民医院就诊的乙型肝炎肝硬化患者327例,检测血清25-羟基维生素D(25-hydroxyvitamin D,25(OH)D)含量,<30 ng/mL者列为实验组,≥30 ng/mL者列为对照组。检测两组血清病毒水平及肝纤维化指标。结果实验组HBV DNA水平明显高于对照组;实验组肝纤维化指标透明质酸(hyaluronic acid,HA)、Ⅲ型前胶原(typeⅢprocollagen,PC-Ⅲ)、Ⅳ型胶原(typeⅣcollagen,C-Ⅳ)明显高于对照组,两组层粘连蛋白(laminin,LN)无显著性差异。结论维持乙肝肝硬化患者的血清维生素D的正常水平,可有利于抑制乙型肝炎病毒复制,延缓肝脏纤维化进程。 Objective To investigate the correlation between serum vitamin D level and hepatitis B virus replication and liver fibrosis in patients with chronic hepatitis B cirrhosis. Methods A total of 327 hepatitis B cirrhosis patients admitted to the sixth people’s hospital of Qingdao from November 2016 to November 2018 were collected. The serum 25-hydroxyvitamin D(25(OH)D) content was detected. Those <30 ng/mL were listed as the experimental group and those ≥ 30 ng/mL as the control group. Serum virus levels and liver fibrosis indexes were detected in the two groups. Results HBV DNA level in the experimental group was significantly higher than that in the control group. Experimental hepatic fibrosis indexes in the experimental group including hyaluronic acid(HA), type Ⅲ procollagen(PC-Ⅲ) and type Ⅳ collagen(C-Ⅳ) were significantly higher than those in the control group. Laminin(LN) of the two groups were not significant different. Conclusion Maintaining the normal level of serum vitamin D in patients with hepatitis B cirrhosis can help to inhibit the replication of hepatitis B virus and delay the process of liver fibrosis.
