Effect of HLA-B27 Gene Expression Changes on the Occurrence of Ankylosing Spondylitis

目的研究强直性脊柱炎中HLA-B27抗原表达变化与基因亚型分布特点及发生发展的影响。方法收集60例强直性脊柱炎患者(早中晚期各20例)及60例参加体检的健康人群的静脉全血,采用流式细胞术检测HLA-B27表达情况。结果HLA-B27的表达与强直性脊柱炎的发生有着显著的关联,且HLA-B2704对强直性脊柱炎的发生发展具有较大的影响,流式细胞术法进行HLA-B27抗原检测诊断强直性脊柱炎的特异性较高;对照组样本HLA-B27表达水平显著低于早中晚期患者表达水平,早中晚期患者间HLA-B27表达水平同样具有统计学差异。结论HLA-B27的表达随着病程的进展有着显著的增加,且基因检测对强直性脊柱炎的灵敏度和特异度均较高,其诊断均具有较高的临床价值。

Objective To investigate the antigen expression changes and gene subtypes distribution of HLA-B27 in the development of ankylosing spondylitis.Methods The venous blood of 60 ankylosing spondylitis patients( 20 cases in the early, middle and late stages,respectively) and 60 healthy people taking medical examinations were collected.Flow cytometry was used to detect the expression of HLA-B27.Results The expression of HLA-B27 was significantly associated with the occurrence of ankylosing spondylitis,and HLA-B2704 had a great influence on the development of ankylosing spondylitis.The specificity of HLA-B27 antigen detection for the diagnosis of ankylosing spondylitis by flow cytometry is higher.The expression level of HLA-B27 in the control group is significantly lower than that in the early,middle and late stage patients and the expression levels of HLA-B27 were also statistically different between patients in the early,middle and late stages.Conclusion The expression of HLA-B27 has a significant increase with the progression of the disease,and the sensitivity and specificity of gene detection for diagnosis of ankylosing spondylitis are high,and has high clinical value.
