通胆汤联合茴三硫治疗慢性胆囊炎的疗效观察

Clinical Observation on Therapeutic Effect of Tongdan Decoction Combined with Anethol Trithione Capsules for Chronic Cholecystitis

【目的】观察童光东教授经验方通胆汤联合茴三硫胶囊治疗慢性胆囊炎的临床疗效。【方法】将82例慢性胆囊炎患者随机分为试验组和对照组,每组各41例。对照组给予茴三硫胶囊治疗,试验组给予通胆汤联合茴三硫胶囊治疗,疗程为3个月。观察2组患者治疗前后腹部胀满、右上腹压痛、恶心嗳气、纳差等中医证候积分的变化情况,通过彩超检测胆囊大小、胆囊壁厚度和胆囊壁毛燥情况以判定2组的影像学疗效,同时评价2组的综合疗效和安全性。【结果】(1)试验组和对照组各有1例患者脱落,实际纳入统计的患者每组各40例。(2)综合疗效方面,治疗3个月后,试验组的总有效率为95.0%,对照组为77.5%,试验组的综合疗效明显优于对照组(P<0.05)。(3)影像学疗效方面,治疗3个月后,试验组的总有效率为97.5%,对照组为80.0%,试验组的影像学疗效明显优于对照组(P<0.05)。(4)治疗后,2组患者的腹部胀满、右上腹压痛、恶心嗳气、纳差等各项中医证候积分均较治疗前明显下降(P<0.01),且试验组的降低作用均明显优于对照组(P<0.05或P<0.01)。(5)治疗期间,2组患者的血常规、心、肝、肾功能指标均在正常范围内,均未出现明显的不良反应。【结论】通胆汤联合茴三硫胶囊治疗慢性胆囊炎具有较好疗效,既能改善患者的临床症状,又能改善患者的影像学指标。

Objective To observe the clinical effect of Tongdan Decoction(the experienced recipe prescribed by Professor TONG Guang-Dong)combined with Anethol Trithione Capsules in the treatment of chronic cholecystitis.Methods Eighty-two patients with chronic cholecystitis were equally randomized into trial group and control group. The control group was treated with Anethol Trithione Capsules, and the trial group was given Tongdan Decoction combined with Anethol Trithione Capsules. Both groups were treated for 3 months. Before and after the treatment, the scores of traditional Chinese medicine(TCM)symptoms such as the abdominal distension and fullness,right epigastric tenderness,nausea and belching,and poor appetite in the two groups were observed.Color Doppler ultrasound was used to detect the gallbladder size,and the thickness and ruffled changes of the gallbladder wall,so as to evaluate the imaging efficacy of the two groups. Moreover,the comprehensive efficacy and safety in the two groups were also evaluated. Results(1)Both of the trial group and control group had one case dropping out, and the final amount of cases in each group was 40.(2)the The comprehensive efficacy: after treatment for 3 months, the total effective rate in the trial group was 95.0% and that in the control group was77.5%,the trial group having stronger comprehensive efficacy than the control group(P<0.05).(3)The imaging efficacy:after treatment for 3 months,the total effective rate in the trial group was 97.5% and that in the control group was 80.0%,the trial group having stronger imaging efficacy than the control group(P<0.05).(4)After treatment, the scores of TCM symptoms such as the abdominal distension and fullness, right epigastric tenderness,nausea and belching,and poor appetite in the two groups were decreased(P<0.01 compared with those before treatment),and the decrease in the trial group was superior to that in the control group(P<0.05 or P<0.01).(5)During the trial,the results of routine blood test and the test for the function of heart,liver and kidney were at normal level,and no obvious adverse reaction was found in the two groups. Conclusion Tongdan Decoction combined with Anethol Trithione Capsules exerts certain effect in the treatment of chronic cholecystitis through relieving the clinical symptoms and improving the imaging indicators.
