梁君昭从痰瘀论治冠心病心绞痛经验

Liang Jun-zhao′s Experience of Treating Angina Pectoris from Phlegm and Blood Stasis

梁君昭教授认为冠状动脉粥样硬化性心脏病(简称冠心病)发作的最主要病机为痰瘀交阻,提出"痰瘀同治、标本兼顾"的理念,以"祛瘀化浊、扶正通络、调畅气机、注重兼症"为治疗法则。诊断要点为追病史,抓舌脉;病症结合,中西互参。治疗要点为痰瘀同治,不忘治本;调畅气机,注重兼症;用药力求精专。常用药对:黄芪、莪术,丹参、山楂,石菖蒲、郁金。梁教授认为临床上不能只拘泥于痰瘀交阻证,常见的还有肝郁气滞、气阴两虚、心肾阳虚等证型。要根据实际症状和舌脉象酌情分析,做到病、证、药三者相统一,方可收到满意的疗效。

Professor Liang Jun-zhao believes that the main pathogenesis of coronary atherosclerotic heart disease(CAHD) is intermingled phlegm and blood stasis stagnation. He puts forward the concept of "treating phlegm and blood stasis together, giving consideration to both the primary and second aspects" and takes "eliminating blood stasis and removing turbidity, strengthening healthy qi and dredging collaterals, regulating qi movement and paying attention to second symptoms" as the treatment principle. His drug use strives for excellence. The commonly-used pair-drugs are as follows:Huangqi(Radix Astragali)-Ezhu(Rhizoma Curcumae);Danshen(Radix et Rhizoma Salviae Miltiorrhizae)-Shanzha(Fructus Crataegi);Shichangpu(Rhizoma Acori Tatarinowii)-Yujin(Radix Curcumae). Professor Liang believes that clinical symptoms can not only be confined to intermingled phlegm and blood stasis stagnation, but also include liver depression and qi stagnation, deficiency of both qi and yin, deficiency of heart and kidney yang, etc. According to the actual symptoms and tongue and pulse conditions, we should analyze them appropriately, and achieve the unity of diseases, syndromes and medicines, so as to achieve satisfactory curative effect.
