经皮微创与切开钢板内固定术治疗四肢骨折的临床效果比较

Clinical Comparison of Percutaneous Minimally Invasive Plate Internal Fixation and Incision Internal Fixation in the Treatment of Limb Fractures

目的比较经皮微创与切开入路钢板内固定术治疗四肢骨折的临床疗效。方法选取2016年6月一2017年12月昌图县第二医院骨外科收治的68例四肢骨折患者,随机分为对照组和观察组各34例。对照组应用传统切开钢板内固定术治疗,观察组选用经皮微创钢板内固定术治疗。比较两组围手术期指标、治疗效果及手术并发症等。结果观察组术中出血量、住院时间均少于对照组,切口长度短于对照组,差异均具有统计学意义(P<0.05)。两组手术和骨折愈合时间差异无统计学意义(P>0.05)。观察组治疗优良率91.2%,略低于对照组的94.1%,差异无统计学意义(χ^2=0,P>0.05)。手术并发症发生率观察组8.8%、对照组29.4%,差异有统计学意义(χ^2=4.660,P<0.05)。结论应用微创钢板内固定术治疗四肢骨折,其效果与传统内固定术相当,但该术式的创伤更小,具有较好的临床应用价值。

Objective To compare the clinical effects of percutaneous minimally invasive plate internal fixation and incision internal fixation in the treatment of limb fractures. Methods A total of 68 cases of limb fracture were selected from June 2016 to December 2017 in Department of Orthopedic Surgery, Second Hospital of Changtu County. All cases were divided into control group and observation group according to random number table method, 34 cases in each group. The control group was treated with traditional incision and internal fixation, while the observation group was treated with percutaneous minimally invasive plate internal fixation. The perioperative indicators, therapeutic effects and operative complications were compared between the two groups. Results The incision length, intraoperative bleeding volume and hospitalization time of the observation group were significantly less than those of the control group (P < 0.05). There was no significant differences in operation time and fracture healing time between the two groups (P > 0.05). The excellent and good rate of the observation group was 91.2%, and slightly lower than 94.1% of the control group (X^2= 0, P > 0.05). The incidence of surgical complications was 8.8% in the observation group, and significantly lower than 29.4% of the control group (X^2 =4.660, P < 0.05). Conclusion The effect of minimally invasive plate internal fixation in the treatment of limb fracture is similar to that of traditional internal fixation, but the operation has less trauma and better clinical application value.
