腰椎间盘突出症采取硬膜下穿刺冲洗的疗效分析

Effect analysis of subdural puncture and irrigation for lumbar ntervertebral disc herniation

目的:探讨腰椎间盘突出症患者采取单纯去椎板减压和去椎板减压并行显微镜下硬膜下穿刺冲洗治疗的临床疗效对比.方法:选取2015年7月-2018年5月我院收治的腰椎间盘突出患者55例,随机分为观察组28例与对照组27例.对照组患者在行神经压迫节段去椎板减压手术过程中,未行硬膜下穿刺冲洗,而观察组患者则在手术过程中接受显微镜下硬膜下穿刺冲洗治疗.使用ASIA评分评价患者的神经功能状态,Barhel指数评价患者的日常生活运动能力的独立性.结果:接受不同治疗3个月后,观察组患者ASIA评分中运动评分、触觉评分和痛觉评分均高于对照组患者,差异有统计学意义(P<0.05).此外,观察组患者Barhel指数亦优于对照组患者(P<0.05).结论:腰椎间盘突出症患者行硬膜下穿刺冲洗,能减少损伤段神经所释放局部的炎症因子,最终帮助损伤的神经恢复,利于患者预后,有一定临床价值.

Objective:purpose Objective: to compare the clinical efficacy of subdural puncture and rinsing for lumbar intervertebral disc herniation with decompression of lamina and decompression of lamina. Methods : fifty-five patients with lumbar intervertebral disc herniation admitted to our hospital from July 2015 to May 2018 were randomly divided into observation group (28 cases) and control group (27 cases). Patients in the control group did not receive subdural puncture and irrigation during nerve compression segmental decompression of vertebral plate surgery, while patients in the observation group received subdural puncture and irrigation under microscope during surgery. The ASIA score was used to evaluate the patients neurological status, and the Barhel index was used to evaluate the independence of the patient's daily life and motor ability. Results: after 3 months of different treatments, the ASIA scores of patients in the observation group were significantly higher than those in the control group ( P < 0. 05 ). In addition, the Barhel index of the patients in the observation group was also better than that of the patients in the control group ( P < 0. 05 ). Conclusion: subdural puncture and irrigation for patients with lumbar intervertebral disc herniation can reduce the local inflammatory factors released by the injured nerves , ultimately help the injured nerve recovery, benefit the prognosis of patients, and have certain clinical value.
