交锁髓内钉联合钢板内固定治疗胫腓骨多段骨折的临床效果及安全性评价

摘要 目的探究交锁髓内钉联合钢板内固定治疗胫腓骨多段骨折的疗效及安全性.方法将2015年6月~2017 年6 月在我院接受治疗的60例胫腓骨多段骨折患者作为研究对象,按治疗方法分组:对照组30例,给予单纯钢板内固定治疗;观察组30 例,给予交锁髓内钉联合钢板内固定治疗,对两组患者手术疗效及安全性进行综合评价.结果观察组患者骨折愈合时间及住院时间均显著短于对照组,手术切口较对照组短(P<0.05),踝关节最大背深度及踝关节功能评分高于对照组(P<0.05 ),两组比较有统计学意义;观察组并发症率6.67%(2/30),对照组23.33%(7/30),两组比较有统计学意义(P<0.05);且观察组患者术后骨折恢复优良率(96.67%),显著高于对照组(80.0%),两组比较有统计学意义(P<0.05).结论交锁髓内钉联合钢板内固定治疗胫腓骨多段骨折,操作简单,安全性高,能够促进骨折愈合,值得应用. Objective To explore clinical efficacy and safety of interlocking intramedullary nailing and plate fix﹣ation for the treatment of segmental fractures of tibia and fibula.Methods 60 patients in our hospital with segmental fractures of tibia and fibula were divided into interlocking intramedullary nailing combined with plate fixation group (observation group)and pure steel plate internal fixation group(control group)from Jun.2015 to Jun.2017.Efficacy and safety of the two groups were evaluated comprehensively.Results The time of fracture healing,hospital stay and surgical incision in observation group were significantly shorter than those in control group(P<0.05).The maximum back depth of ankle and functional score of the ankle joint in observation group were significantly higher than those in control group(P<0.05).There was significant difference on complication rate between two groups with 6.67%(2/30)in observation group and 23.33%(7/30)in control group(P<0.05 ).The recovery rate of fracture recovery in observation group(96.67%)was significantly higher than that in control group(80.0%)(P<0.05 ).Conclusion Interlocking intramedullary nailing combined with plate fixation can promote fracture healing in the treatment of seg﹣mental fractures of tibia and fibula with simple operation and high safety.
