益气升降汤治疗冠心病心绞痛宗气亏虚证的临床研究

Clinical study on angina pectoris from coronary heart disease with a type of deficiency of pectoral qi treated by Yiqi Shengjiang decoction

目的:探讨益气升降汤治疗冠心病心绞痛宗气亏虚证的临床疗效及安全性。方法:38例患者在常规治疗基础上给予益气升降汤口服,疗程为4周。观察治疗后中医症状、中医证候、心绞痛疗效、心电图变化、硝酸甘油停减及不良反应。结果:中医证候总有效率为92.1%;治疗前中医证候总得分为(19.08±3.60)分,高于治疗后的(9.92±4.49)分,差异有统计学意义(P<0.05)。中医主症方面,治疗后胸闷(胸痛)、太息等主症均明显改善,总有效率分别为89.5%、92.1%;中医主症胸闷(胸痛)得分为(1.53±1.50)分,低于治疗前的(4.11±1.31)分,太息得分为(1.47±1.45)分,低于治疗前的(3.47±1.37)分,差异均有统计学意义(P<0.05)。西医疗效方面,心绞痛总有效率为94.7%。治疗后,疼痛程度、发作次数、持续时间、硝酸甘油用量等心绞痛症状得分均低于治疗前,差异有统计学意义(P<0.05)。心电图显示总有效率为46.7%;硝酸甘油停减率为82.8%;治疗过程中未出现不良反应。结论:益气升降汤治疗冠心病心绞痛疗效确切,可有效改善冠心病心绞痛临床症状,且安全无不良反应,值得临床推广应用。

Objective:To invrstigate the clinical efficacy and safety of Yiqi Shengjiang decoction on angina pectoris from coronary heart disease,with a type of deficiency of pectoral qi.Methods:Based on the normal treatment,38 cases were given Yiqi Shengjiang decoction.The treatment course was 4 weeks.After treatment,the symptoms of traditional Chinese medicine(TCM),angina pectoris curative effect,changes of electrocardiogram,nitroglycerin reduction,as well as the adverse reaction were observed.Results:The total effective rate of TCM symptoms was 92.1%.TCM syndrome score before treatment was(19.08±3.60),which was higher than that after treatment(9.92±4.49),the differences was statistically significant(P<0.05).Talking about main symptoms of TCM,chest tightness,chest pain and sigh were all obviously improved.The total efficiency was respectively 89.5% and 92.1%.After treatment,the score of chest tightness or chest pain was(1.53±1.50),which was lower than that before treatment(4.11±1.31).The score of sigh was(1.47±1.45),which was lower than that before treatment(3.47±1.37),and both differences were statistically significant(P<0.05).Talking about the western medicine curative effect,the total effective rate of angina pectoris was 94.7%.The score of the degree of pain,number of episodes,duration,and dosage of nitroglycerin were lower than before the treatmen,and the differences were statistically significant(P <0.05).The total effective rate of electrocardiogram was 46.7%.The reduction rate of nitroglycerine was 82.8%.No adverse reactions occurred during the treatment.Conclusion:To treat angina pectoris from coronary heart disease,with a type of deficiency of pectoral qi,by Yiqi Shengjiang decoction can receive exact effect,which can improve the clinical symptoms of angina pectorisis.So it is worthy of clinical popularization and application.
