MRE检查对克罗恩病活动度的诊断价值

Diagnostic value of MRE for the evaluation of Crohn’s disease activity

[目的]探讨血清学检查、内镜检查以及磁共振活动度指数(MRE)检查对克罗恩病(CD)活动度的诊断效率。[方法]纳入68例CD患者,收集其CRP、ESR、SES-CD评分、MaRIA评分、简化CDAI评分,分析上述指标对CD活动度的诊断效率。[结果]:总SES-CD对CD活动度的诊断率为76.6%,灵敏性62.5%,特异性83.3%,cut-off值为11.5;总MaRIA对CD活动度的诊断效率为71.2%,灵敏性80.0%,特异性67.6%,cut-off值为31.18;MaRIA最高分对CD活动度的诊断效率为65.5%,灵敏性63.5%,特异性79.4%,cut-off值为22.2;总MaRIA对中度活动度的诊断效率为77.9%,灵敏性83.3%,特异性71.2%,cut-off值为38.75;MaRIA最高分对中度活动度的诊断效率为69.8%,灵敏性66.7%,特异性67.3%,cut-off值为22.5(P<0.05)。[结论]MRE检查对CD活动度尤其是中度活动度的诊断效率较高。

[Objective]To investigate the diagnostic efficiency of serological examination,endoscopy and magnetic resonance enterography(MRE)examination for Crohn’s disease(CD)activity.[Methods]Sixty-eight patients with CD admitted to Zhongnan Hospital of Wuhan University from July 2016 to September 2018 were enrolled.CRP,ESR,SES-CD score,MaRIA score,simplified CDAI score were collected,and the diagnostic efficiency of CDAI activity was analyzed.[Results]Total SES-CD had a diagnostic efficiency of CD activity of 76.6%,sensitivity of 62.5%,specificity of 83.3%,and cut-off value of 11.5.The diagnostic efficiency of total MaRIA for CD activity was 71.2%,sensitivity was 80%.Specificity was 67.6%,cut-off value was 31.18;MaRIA highest score had a diagnostic efficiency of 65.5% for CD activity,sensitivity of 63.5%,specificity of 79.4%,and cut-off value of 22.2;total MaRIA for moderate activity.The diagnostic efficiency was 77.9%,the sensitivity was 83.3%,the specificity was 71.2%,and the cut-off value was 38.75.The diagnostic efficiency of the highest score of MaRIA for moderate mobility was 69.8%,the sensitivity was 66.7%,the specificity was 67.3%,and the cut-off value was 22.5(P<0.05).[Conclusion]MRE examination is more effective in diagnosing CD activity,especially in moderate activity.
