血清胃泌素-17、癌胚抗原及胃蛋白酶原对胃癌的诊断价值分析

Diagnostic value of serum G-17, CEA and PG in the diagnosis of gastric cancer

目的探讨血清胃泌素-17(G-17)、癌胚抗原(CEA)、胃蛋白酶原(PG)对胃癌的诊断价值。方法选择80例胃癌患者和80例胃炎患者,分别作为胃癌组和对照组,检测并比较两组患者的血清G-17、CEA、PGⅠ及PGⅡ水平。以病理诊断结果为金标准,分析血清G-17、CEA、PGⅡ单独及联合检测对胃癌的诊断价值。结果胃癌组患者的血清G-17、CEA及PGⅡ水平均明显高于对照组,差异均有统计学意义(P﹤0.01);两组患者的血清PGⅠ水平比较,差异无统计学意义(P﹥0.05)。以病理诊断结果为金标准,血清G-17+CEA+PGⅡ联合诊断胃癌的灵敏度为88.75%,特异度为80.00%,准确度为84.38%,约登指数为0.688,均高于任何单一指标的诊断结果。结论血清G-17、CEA及PGⅡ联合检测对胃癌具有一定的诊断价值,值得在临床推广。

Objective To explore the clinical value of serum gastrin-17(G-17),carcinoembryonic antigen(CEA)and pepsinogen(PG)in the diagnosis of gastric cancer.Method A total of 80 patients with gastric cancer and another 80 cases with gastritis were included in the study as gastric cancer group and control group,respectively,of which the serum G-17,CEA and PGⅠand PGⅡwere detected and compared.With pathological findings as the gold standard,the clinical and diagnostic value of serum G-17,CEA,and PGⅡutilized alone or in combination for the diagnosis of gastric cancer were evaluated.Result The serum levels of G-17,CEA and PGⅡin gastric cancer group were significantly higher than those in control group(P<0.05);while there was no significant difference between gastric cancer group and control group in regard of serum PGⅠlevel(P>0.05).With pathological results referenced as the diagnostic gold standard,the sensitivity of combined detection using serum G-17+CEA+PG II was 88.75%,specificity was 80.00%,and accuracy was 84.38%,with the Yoden index being 0.688,all were higher compared to the results obtained as each marker applied alone.Conclusion The combined detection of serum G-17,CEA and PGⅡis relatively beneficial in the diagnosis of gastric cancer.
