抵抗素基因-420多态性及其血清学水平与川崎病的关系

摘要 目的探讨抵抗素基因启动子-420C/G多态性与川崎病(Kawasaki disease,KD)的关系,及其对血清抵抗素水平的影响。方法选择81例川崎病患儿(KD组),其中11例并发冠脉损伤(coronary artery lesion,CAL)和100例正常儿童为正常对照组,采用基因测序检测不同组别抵抗素基因-420C/G位点多态性,酶联免疫方法(ELISA)测定空腹血清抵抗素水平。结果(1)KD组抵抗素基因-420C/G的等位基因频率及CC、CG、GG基因型分布与正常对照组比较差异无统计学意义(χ~2=1.294、0.188,P=0.524、0.665)。(2)KD组中并CAL者与无CAL者基因型分布频率和等位基因频率比较差异亦无统计学意义(χ~2=0.621、0.234,P=0.838、0.628)。(3)KD组的血清抵抗素水平明显高于正常对照组(t=3.725,P <0.01)。合并CAL血清抵抗素水平与未合并CAL组比较差异无统计学意义(t=0.730,P=0.467)。KD组抵抗素基因-420基因型CC、GG、CG血清抵抗素比较差异无统计学意义(F=1.785,P=0.175)。正常对照组抵抗素基因-420基因型CC、CG、GG血清抵抗素比较差异无统计学意义(F=0.544,P=0.582)。结论尚未发现抵抗素基因-420C/G位点与川崎病及冠脉损伤发生相关,同时,川崎病与正常儿童中抵抗素基因-420多态性与抵抗素血清学水平亦不相关。但KD患儿血清抵抗素与KD发生有关。 Objective This study investigated the association of the resistin(RETN)promoter-420 C/G polymorphism with Kawasaki disease(KD)and its serum resistin level in Chinese children. Methods A total of 81 children with Kawasaki disease(among whom 11 had CAL)and 100 normal children who underwent physical examination(control group)were enrolled in a case-control study. Genotyping of the RETN promoter polymorphism was performed using gene-sequencing technology,and serum resistin levels were estimated using the enzyme immunoassay method. Results There were no significant differences in C/G alleles and the frequencies of CC,CG,and GG genotypes of-420 C/G polymorphism in the RETN gene between the KD and control groups(P > 0.05).Meanwhile,in the Kawasaki disease group,there was no significant difference in RETN(-420 C > G)genotypes between the children with CAL and those without. Serum levels of RETN were significantly higher in KD patients than in controls regardless of the presence of coronary artery lesions(CALs). Both in KD and control groups,no significant differences were found between the Serum levels of RETN in CC,CG,and GG genotypes of-420 C/G polymorphism. Conclusions The RETN-420 C/G polymorphism may not be associated with Kawasaki disease or CAL. Also there is no apparent association on the RETN(-420 C > G)gene polymorphism between KD and Chinese children. However,serum resistin level significantly increase in KD,which suggests RETN may play a role in the pathogenesis of KD.
