临床药师对化疗后出现Ⅳ度血小板减少症患者全方位药学监护服务

Clinical pharmacist’s comprehensive pharmaceutical care for patients with Ⅳ-level thrombocytopenia after multi-line chemotherapy

目的探讨当患者化疗后出现Ⅳ度血小板减少等严重骨髓抑制时,临床药师如何进行科学的药学监护。方法通过对患者进行不良反应发生的原因分析、用药选择的指导、后续复发及预防提醒和化疗方案的调整建议,为患者提供全方位的药学监护。结果患者得到悉心治疗,血小板等指标恢复正常。结论临床药师针对患者化疗的并发症,围绕血小板减少来实施个体化的药学监护,可明显减轻血小板减少等带来的危害。

Objective To explore how clinical pharmacists conduct pharmaceutical care when patients have myelosuppression such as Ⅳ-degree thrombocytopenia after chemotherapy. Methods Clinical pharmacists provided patients with comprehensive pharmaceutical care through analysis of the causes of adverse reactions to patients,guidance of medication selection,follow-up recurrence and prevention reminders,and recommendations for adjusting the chemotherapy. Results The patient was treated carefully and the platelet values returned to normal. Conclusion Clinical pharmacists perform individualized pharmaceutical care around thrombocytopenia in response to complications of chemotherapy. This significantly reduces the harm of thrombocytopenia and so on.
