探讨精细化护理对下肢骨折患者术后康复的影响

Exploring the Effect of Refined Nursing on Postoperative Rehabilitation of Patients with Lower Extremity Fracture

目的研究精细化护理对下肢骨折患者术后康复的影响,以此为下肢骨折患者制定科学的护理方案。方法方便选择该院骨科2017年3月—2018年5月临床收治的116例下肢骨折患者作为入选研究对象,将116例下肢骨折患者分成对照组(n=58,给予常规护理)与观察组(n=58,给予精细化护理),比较不同护理措施应用后对两组患者术后康复的影响。结果观察组导尿管留置时间(16.2±4.4)h短于对照组(19.2±4.5)h(t=6.2009,P=0.0127);观察组下床活动时间(3.5±1.1)d短于对照组(4.6±1.4)d(t=7.8622,P=0.0050);观察组住院时间(8.8±2.0)d短于对照组(11.6±2.8)d(t=7.8431,P=0.0051);护理后,观察组HAMA(7.2±1.7)分、HAMD(10.1±3.1)分评分优于对照组HAMA(10.8±3.3)分、HAMD(16.5±3.2)分(t=13.0909、t=7.3846,P=0.0002、0.0065)差异有统计学意义(P<0.05)护理前,两组Fugl-Mcyer、ADL评分差异无统计学意义(P>0.05);护理后优于护理前,观察组优于对照组,差异有统计学意义(P<0.05);观察组满意度98.28%高于对照组82.76%(χ^2=8.1351,P=0.0043)。结论精细化护理应用于下肢骨折患者能有效改善患者术后康复情况,值得推广应用。

Objective To study the effect of refined nursing on postoperative rehabilitation of patients with lower limb fractures, and to develop a scientific nursing plan for patients with lower limb fractures. Methods Convenient select a total of 116 patients with lower extremity fractures who were admitted to our department from March 2017 to May 2018 were enrolled. 116 patients with lower extremity fractures were divided into control group (n=58, given routine care) and observation group (n=58, given refined care), comparing the effects of different nursing measures on the postoperative rehabilitation of the two groups of patients. Results The catheter indwelling time (16.2±4.4) h was shorter in the observation group than in the control group (19.2±4.5)h(t=6.200 9, P=0.012 7);the time to get out of bed in the observation group (3.5±1.1)d was shorter than that in the control group(4.6±1.4)d(t=7.862 2, P=0.005 0);the hospitalization time (8.8±2.0)d in the observation group was shorter than that in the control group (11.6±2.8)d(t=7.843 1, P=0.005 1);after the nursing, the observation group HAMA (7.2±1.7)points and HAMD (10.1±3.1)points were better than those of the control group, HAMA (10.8±3.3)points, and HAMD (16.5±3.2)points (t=13.090 9,t=7.384 6, P=0.006 5);there was statistically significant difference(P<0.05) before treatment, there was no statistically significant difference in Fugl-Mcyer and ADL scores between the two groups(P>0.05). After nursing, the observation group was superior to the control group. The contrast was statistically significant(P<0.05);the observation group satisfaction was 98.28% higher than the control group 82.76%(χ^2=8.135 1, P=0.004 3). Conclusion The application of refined nursing in patients with lower extremity fracture can effectively improve the postoperative rehabilitation of patients, which is worthy of popularization and application.
