竖脊肌平面阻滞在麻醉与疼痛中的临床应用进展 被引量:12

摘要 竖脊肌平面阻滞是近年来麻醉学和疼痛诊疗学应用的新型躯干神经阻滞方法。该方法药物注射的目标位置位于竖脊肌与椎体横突之间,为一肌肉间隙,结合超声引导,其定位及操作简单,一经提出在临床上得到了广泛应用。基于竖脊肌较长的走行,不同的进针平面可得到相应的镇痛节段,在围手术期镇痛如胸部手术、腹部手术、骨科手术中得以应用;与其他神经阻滞技术相同,阻断疼痛的外周传导作用使其在疼痛学中也有广泛应用,如开胸术后疼痛综合征、慢性肩痛、复杂区域疼痛综合征等。 Erector spinae plane block( ESPB) is a new method of trunk nerve block in anesthesiology and pain medicine in recent years. The target position of the drug injection is a muscle space between the erector spinae muscle and the transverse process of the vertebral body,and it has been widely used in clinical,because of the simple location and operation,high safety with the guidance of ultrasound. Based on the broad coverage of the erector spinae muscle,the analgesic level could be obtained at different needle planes,which could be used in perioperative analgesia such as thoracic surgery,abdominal surgery and orthopedic surgery. Similar to the other nerve block techniques,the blocking of peripheral conduction of pain makes it widely used in pain studies such as pain syndrome after thoracotomy,chronic shoulder pain,complex area pain syndrome and so on.
