幽门螺杆菌感染与炎症性肠炎关系及危险因素分析

Analysis of the Relationship Between Helicobacter Pylori Infection and Inflammatory Bowel Disease and the Risk Factors

目的探讨幽门螺杆菌(HP)感染与炎症性肠炎关系及危险因素。方法回顾性分析2017年2月至2019年2月成都医学院第一附属医院收治的初诊炎症性肠炎患者100例及同期健康体检者100例。收集一般资料、HP感染情况和血清炎性因子水平。以多因素Logistic回归分析炎症性肠炎影响因素,并分析HP感染与炎症因子水平的相关性。结果HP感染为炎症性肠炎保护性因素,C-反应蛋白(CRP)≥5mg/L、白介素-6(IL-6)≥25ng/L、降钙素原(PCT)≥0.5μg/L、肿瘤坏死因子(TNF-α)≥30fmol/mL为危险因素(P<0.05)。HP感染与CRP、IL-6、PCT及TNF-α水平均呈负相关(r=-0.613、-0.727、-0.593、-0.810,P<0.05)。结论HP感染为炎症性肠炎保护性因素,且炎症性肠炎患者HP感染与CRP、IL-6、PCT及TNF-α水平均呈负相关。

Objective To investigate the relationship between Helicobacter pylori (HP) infection and inflammatory bowel disease and analyze the risk factors. Methods The clinical data of 100 patients with newly-diagnosed inflammatory bowel disease treated in The First Affiliated Hospital of Chengdu Medical College from February of 2017 to February of 2019 and the other 100 healthy controls undergoing the physical examination over the corresponding period were analyzed retrospectively. The data of the general information, HP infection and serum inflammatory factor were collected. The multivariate logistic regression analysis was used to analyze the influencing factors of inflammatory bowel disease, and the correlation between HP infection and inflammatory factors was analyzed. Results HP infection was a protective factor for inflammatory bowel disease. The factors including CRP≥5mg/L, IL-6≥25ng/L, PCT≥0.5μg/L and TNF-α≥30fmol/ml were the risk factors ( P< 0.05). HP infection was negatively correlated with the levels of CRP, IL-6, PCT and TNF-α( r values were -0.613,-0.727, 0.593 and -0.810 respectively, P< 0.05). Conclusion HP infection is a protective factor for inflammatory bowel disease, and HP infection is negatively correlated with the levels of CRP, IL-6, PCT and TNF-α in patients with inflammatory bowel disease.
