胚胎型大脑后动脉与后循环脑梗死的关系 被引量:2

The relationship between fetal-type posterior cerebral artery and the cerebral infarction of posterior circulation

摘要 目的:探讨胚胎型大脑后动脉(fetal-type posterior cerebral artery, FTP)与后循环脑梗死的关系。方法:收集2015年9月至2017年10月在贵州医科大学附属医院神经内科住院治疗的脑梗死患者。所有患者均行颅脑MRI+ DWI+ MRA检查。根据MRA结果将研究对象分为FTP组和非FTP组。根据英国牛津郡社区脑卒中项目(OCSP)的分型标准及影像学检查结果,将脑梗死患者梗死部位按血管分布分为前循环脑梗死和后循环脑梗死。探讨FTP与后循环脑梗死的相关性。结果:共556例脑梗死患者纳入研究,其中男性355例,女性201例;前循环脑梗死患者331例,后循环脑梗死患者225例。共有208例患者诊断为FTP,其中单侧FTP患者148例,双侧FTP患者60例。前循环脑梗死中FTP患者104例,后循环脑梗死中FTP患者104例。FTP检出率在前循环脑梗死和后循环脑梗死两组患者之间差异有统计学意义(P<0. 001)。糖尿病是后循环脑梗死的危险因素。结论:脑梗死患者中FTP出现率高,单侧FTP更常见;合并FTP的糖尿病患者,更易发生后循环脑梗死;FTP与后循环脑梗死的发生有一定的相关性。 Objective: To research the relationship between fetal-type posterior cerebral artery and cerebral infarction of posterior circulation. Methods: The consecutive patients with cerebral infarction, hospitalized from September 2015 to October 2017 were collected. All patients underwent MRI、DWI and MRA. They were divided into either FTP group or non-FTP group according to the results of magnetic resonance imaging. Based on the classification criteria of OCSP and imaging results, the infarction sites of patients with cerebral infarction were divided into anterior circulation ischemia stroke and posterior circulation ischemia stroke according to the vascular distribution. The correlation between PTF and posterior circulation ischemia stroke was analyzed. Results: A total of 556 patients with cerebral infarction were enrolled , including 331 cases in the anterior circulation group and 225 in the posterior circulation group. 208 people were diagnosed as cases with FTP. The detection rate of FTP was higher than that of FTP between the posterior circulation group and the anterior circulation group, and the difference was statistically significant(P<0. 001). Diabete was the risk factor for posterior circulation ischemia stroke. Conclusion: The incidence of FTP in patients with cerebral infarction is high, one-sided FTP is more comm on. Diabetics with FTP are more likely to have posterior circulation ischemia stroke. FTP is correlated with the incidence of posterior circulation ischemia stroke.
