模拟重力状态下MRI检查诊断腰椎间盘突出症的价值 被引量:8

摘要 目的通过自行设计的装置,在常规平卧位腰椎MRI检查时予以模拟重力状态的轴向负荷,评价其对椎间盘突出症的诊断价值。方法将研究对象按照是否存在腰椎间盘突出症分为正常组和病例组,正常组40例,病例组46例。常规仰卧位及负荷仰卧位下行MRI检查,观察L4、5及L5S1节段腰椎间盘高度、椎间盘突出程度、硬膜囊面积的变化。结果正常组负荷仰卧位时L4、5节段椎间盘高度低于常规仰卧位,差异有统计学意义(P<0.05)。病例组常规仰卧位与负荷仰卧位时L4、5、L5S1节段椎间盘高度差异无统计学意义(P>0.05)。病例组负荷仰卧位时L4、5节段椎间盘突出程度较常规仰卧位时严重,差异有统计学意义(P<0.05)。病例组常规仰卧位时与负荷仰卧位时L5S1节段椎间盘突出程度差异无统计学意义(P>0.05)。病例组负荷仰卧位时L4、5与L5S1节段硬膜囊面积小于常规仰卧位,差异有统计学意义(P<0.05)。结论采用自行设计的装置可以在常规平卧位MRI检查时予以轴向应力,模拟患者站立位时的状态,能提供更多有价值的诊断信息,可提高诊断椎间盘突出症的敏感性。
