摘要
目的探讨血清胱抑素C(CysC)、尿酸(UA)对急性脑梗死患者的临床意义。方法选取2019年1~5月收治入我院的80例急性脑梗死患者作为观察组,进行回顾性分析,选择同期进行健康体检者60例为对照组,分别检测并比较两组血清CysC和UA的水平,分析两者对急性脑梗死患者的临床价值。结果观察组血清CysC和UA水平较对照组均显著升高(P<0.05);血清CysC和UA的ROC曲线下面积分别为0.803、0.619,CysC的诊断价值中等,最佳诊断界值时对应的敏感度和特异性分别为71.3%、73.3%,阳性预测值和阴性预测值分别为78.1%、65.7%,UA的筛查和诊断价值偏低,最佳诊断界值时对应的敏感度和特异性分别为47.5%、75.0%,阳性预测值和阴性预测值分别为71.7%、51.7%。结论CysC对急性脑梗死有一定的诊断价值,与病情发生发展密切相关,而UA对急性脑梗死的筛查和诊断价值偏低,可作为急性脑梗死的筛查指标。
Objective To investigate the clinical significance of detection of serum cystatin C(CysC)and uric acid(UA)in the patients with acute cerebral infarction.Methods 80 patients with acute cerebral infarction who were admitted to our hospital from January to May 2019 were selected in the observation group for retrospective analysis.60 subjects who were given healthy physical examination at the same period were selected in the control group.The levels of serum CysC and UA in the two groups were detected and compared,and the clinical value of both indices was analyzed in the patients with acute cerebral infarction.Results The levels of serum CysC and UA in the observation group were significantly higher than those in the control group(P<0.05);the area under the ROC curve of serum CysC and UA was 0.803 and 0.619,respectively,and the diagnostic value of CysC was medium.The sensitivity and specificity of the optimal diagnostic cutoff were 71.3%and 73.3%,respectively.The positive predictive value and negative predictive value were 78.1%and 65.7%,respectively.The value of screening and diagnosis of UA was low.The sensitivity and specificity of the optimal diagnostic cutoff were 47.5%and 75.0%,respectively.The positive predictive value and the negative predictive value were 71.7%and 51.7%,respectively.Conclusion CysC has a certain diagnostic value for acute cerebral infarction,which is closely related to the occurrence and development of the disease.The value of UA for the screening and diagnosis of acute cerebral infarction is low,which can be used as a screening index for acute cerebral infarction.
