经阴道超声联合经腹超声诊断宫颈癌的临床研究

Clinical study of transvaginal color Doppler ultrasound combined with transabdominal color Doppler ultrasound in the diagnosis of crvical cancer

目的评价经阴道彩色多普勒超声(TV-CDU)联合经腹部彩色多普勒超声(TA-CDU)诊断宫颈癌的临床价值。方法选择2017年1月~2018年8月来我院肿瘤妇科门诊与住院疑诊为宫颈癌的患者113例为本次研究对象。113例患者均分别行TV-CDU、TA-CDU、TV-CDU联合TA-CDU检查,以病理组织学为金标准,计算3种诊断方法的符合率。结果诊断宫颈癌符合情况:TA-CDU诊断符合率为53.98%,TV-CDU诊断符合率为79.65%,联合检查诊断符合率为95.58%,3种诊断方法符合率均存在明显差异(P<0.01);诊断宫颈癌临床分期符合情况:无论是TA-CDU还是TV-CDU诊断Ⅰ期宫颈癌均存在一定不足,只能通过宫颈出现肥大进行分段诊刮与阴道镜深入检查。Ⅱ期及以上宫颈癌联合检查诊断符合率明显提升。结论TV-CDU联合TA-CDU诊断宫颈癌符合率较高,且与临床分期诊断符合率较高,对宫颈癌诊断及分期诊断可提供科学依据。

Objective To evaluate the clinical value of transvaginal color Doppler ultrasound(TV-CDU)combined with transabdominal color Doppler ultrasound(TA-CDU)in the diagnosis of cervical cancer.Methods A total of 113 patients who were suspected to have cervical cancer in the outpatient and inpatient of Gynecological Oncology in our hospital from January 2017 to August 2018 were selected for this study.All the 113 patients were examined by TV-CDU,TA-CDU,and TV-CDU combined with TA-CDU respectively,and the pathological histology was used as the gold standard to calculate the coincidence rate of the three diagnostic methods.Results The coincidence rate of cervical cancer diagnosis:the coincidence rate of TA-CDU diagnosis was 53.98%,the coincidence rate of TV-CDU diagnosis was 79.65%,and the coincidence rate of combined diagnosis was 95.58%.The coincidence rates of the three diagnostic methods were significantly different(P<0.01).The coincidence rate of cervical cancer staging diagnosis:Both TA-CDU and TV-CDU had a certain deficiency in the diagnosis of stageⅠcervical cancer,which could only detect the hypertrophic segment and fractional curettage and colposcopy were needed.In stageⅡand above cervical cancer,the coincidence rate of combined examination was significantly improved.Conclusion The coincidence rate of TV-CDU combined with TA-CDU in the diagnosis of cervical cancer is higher,and the coincidence rate with clinical staging diagnosis is higher,which can provide a scientific basis for the diagnosis and staging diagnosis of cervical cancer.
