集束化护理干预预防新生儿呼吸机相关性肺炎的价值分析

Analysis on the value of cluster nursing intervention for the prevention of neonatal ventilator-associated pneumonia

目的探讨集束化护理干预预防新生儿呼吸机相关性肺炎的临床效果。方法随机抽取2017年4月~2019年1月我院收集的呼吸机相关性肺炎新生儿72例,分为对照组和研究组两组,各36例,对照组使用常规护理干预,研究组使用集束化护理干预,比较两组的通气时间、治疗时间、呼吸机相关性肺炎的发生率PO2、PCO2、FEV1/FVC以及护理满意度。结果研究组的肺功能水平以及护理总满意度均高于对照组(P<0.05),且通气时间[(6.53±2.01)d]和治疗时间[(8.54±2.59)d]、呼吸机相关性肺炎的发生率(30.56%)均低于对照组(t=5.0119、6.3442,χ^2=5.6250,P<0.05)。结论在预防新生儿呼吸机相关性肺炎时使用集束化护理干预,可以降低呼吸机相关性肺炎的发生率,改善新生儿的肺功能,促进新生儿康复,并缩短了通气时间和治疗时间,提高家属对护理的满意度,改善医患关系,因此可推广使用。

Objective To investigate the clinical effects of cluster nursing intervention for the prevention of neonatal ventilator-associated pneumonia.Methods A total of 72 neonates of neonatal ventilator-associated pneumonia were collected by random selection method from April 2017 to January 2019 in our hospital.They were assigned to the control group and study group,with 36 cases in each group.The control group were given routine nursing intervention,and the study group were given cluster nursing intervention.Ventilation time,treatment time,incidence rate of ventilator-associated pneumonia,PO2,PCO2,FEV1/FVC,and nursing satisfaction were compared between the two groups.Results The lung function levels and total nursing satisfaction in the study group were higher than those in the control group(P<0.05),and the ventilation time[(6.53±2.01)d],treatment time[(8.54±2.59)d],and the incidence rate of ventilator-associated pneumonia(30.56%)were all lower than those in the control group(t=5.0119,6.3442,χ^2=5.6250,P<0.05).Conclusion The use of cluster nursing intervention in the prevention of neonatal ventilator-associated pneumonia can reduce the incidence rate of ventilator-associated pneumonia,improve the lung function of newborns,and promote neonatal rehabilitation.It also shortens the ventilation time and treatment time,improves the family's satisfaction with nursing,and improves the relationship between doctors and patients.Therefore,it can be promoted and applied.
