后外侧入路联合内侧入路内固定治疗三踝骨折临床分析

Clinical Analysis of Posterior Lateral Approach Combined with Medial Approach for the Treatment of Trigeminal Fracture

目的探究三踝骨折患者采取后外侧入路、内侧入路联合内固定术治疗的临床价值。方法该次研究针对目标为2013年1月—2018年12月期间该院骨科收治40例三踝骨折患者,参照随机抽选结果将其分为观察组与对照组,对照组采用内外侧入路联合前侧闭合螺钉内固定治疗,观察组采用后外侧入路联合内侧入路内固定治疗,对比两组患者总疗效,统计并分析治疗前后患者肢体功能恢复情况,计算并发症发生概率。结果观察组总疗效95.00%明显优于对照组65.00%,差异有统计学意义(χ^2=5.63,P<0.05),且经一段时间恢复后,观察组患者肢体屈伸评分(7.69±0.47)分、内外翻转评分(7.58±0.22)分、足部对线评分(7.55±0.13)分均优于对照组,疼痛评分(3.01±0.17)分低于对照组,差异有统计学意义(t=15.88、34.28、61.17、44.88,P<0.05)。观察组患者出现并发症的概率更低。结论三踝骨折患者以后外侧入路联合内侧入路内固定治疗效果更佳,可改善患者肢体运动功能,且术后患者并发症更少,值得推广应用。

Objective To investigate the clinical value of posterior lateral approach and medial approach combined with internal fixation for patients with trigeminal fracture. Methods In this study, 40 patients with trigeminal fractures were enrolled in the orthopedic department of this hospital from January 2013 to December 2018. The results were randomly divided into observation group and control group, and the control group was treated with internal and external ap-proaches combined with anterior closed screw internal fixation, the observation group was treated with posterolateral approach and medial approach. The total efficacy of the two groups was compared. The recovery of limb function before and after treatment was analyzed and the probability of complications was calculated. Results The total efficacy of the observation group was 95.00%, which was significantly better than that of the control group 65.00%,the difference was statistically significant(χ^2 =5.63, P<0.05). After recovery for a period of time, the limb flexion and extension score (7.69± 0.47)points and internal and external turnover score (7.58±0.22)points were observed in the observation group, the foot-to-line score (7.55±0.13)points was better than the control group, and the pain score (3.01±0.17)points was lower than the control group,the difference was statistically significant (t=15.88, 34.28, 61.17, 44.88, P<0.05). Patients in the ob-servation group had a lower probability of complications. Conclusion The posterior lateral approach combined with the medial approach for internal fixation of the three-ankle fracture is better, which can improve the limb motor function of the patient, and the postoperative complications are less. It is worthy of popularization and application.
