猪肺磷脂辅助治疗早产儿呼吸窘迫综合征疗效评价

Efficacy of Poractant Alfa Injection in the Adjuvant Treatment of Respiratory Distress Syndrome in Premature Infants

目的探讨猪肺磷脂辅助治疗早产儿呼吸窘迫综合征(RDS)的临床疗效。方法选取湖北省应城市人民医院2016年1月至2017年12月收治的RDS早产儿100例,按随机数字表法分为试验组和对照组,各50例。两组患儿均予经鼻持续气道正压通气(NCPAP)、电解质平衡等支持治疗,试验组患儿加用猪肺磷脂注射液,均治疗48 h。结果治疗后,两组患儿动脉血氧分压(PaO2)、p H、血氧饱和度(SaO2)、心输出量(CO)、心肌收缩指数(ICON)、左室射血分数(LVEF)、每搏输出量(SV)均显著高于对照组(P<0.05),动脉血二氧化碳分压(PaCO2)显著低于对照组(P<0.05),NCPAP通气时间、氧疗时间、住院时间均显著短于对照组(P<0.05),且试验组改善程度均优于对照组(P<0.05)。结论猪肺磷脂辅助治疗早产儿RDS,能进一步改善患儿的缺氧症状,同时提高心功能,缩短治疗时间。

Objective To investigate the clinical efficacy of poractant alfa in the adjuvant treatment of respiratory distress syndrome(RDS) in premature infants.Methods Totally 100 cases of RDS premature infants admitted to Yingcheng People's Hospital from January 2016 to December 2017 were selected and divided into the experimental group and the control group by the random number table method,50 cases in each group.The patients in the two groups were treated with nasal continuous positive airway pressure(NCPAP)and electrolyte balance, on this basis, patients in the experimental group were treated with Poractant Alfa Injection.Both groups were treated for 48 h.Results After treatment,the arterial partial pressure of oxygen(PaO2),pH,oxygen saturation(SaO2),cardiac output(CO),myocardial contraction index(ICON),left ventricular ejection fraction(LVEF) and stroke output(SV) in the experimental group were significantly higher than those in the control group(P<0.05),while the arterial partial pressure of carbon dioxide(PaCO2) in the experimental group was significantly lower than that in the control group(P<0.05),the NCPAP ventilation time,oxygen therapy time and hospitalization time in the experimental group were significantly shorter than those in the control group(P<0.05),and the improvement of the experimental group was better than that of the control group(P<0.05).Conclusion Poractant alfa in the adjuvant treatment of RDS in premature infants can further improve the symptoms of hypoxia and heart function,it also can shorten the treatment time.
