血清甲状腺过氧化物酶抗体在自身免疫性甲状腺疾病诊断中的应用价值研究

The Application Value of Serum Thyroid Peroxidase Antibody in the Diagnosis of Autoimmune Thyroid Diseases

目的分析评价血清甲状腺过氧化物酶抗体在自身免疫性甲状腺疾病诊断中的应用价值。方法此次研究随机抽取了在该院治疗的78例自身免疫性甲状腺疾病患者作为研究对象,病例数选取时间为2016年8月—2018年12月,其中原发性甲状腺功能紊乱患者20例,桥本甲状腺炎患者29例,Graves病患者29例,同时选取30名于该院进行体检的健康志愿者作为对照组,比较自身免疫性甲状腺疾病患者和对照组的血清甲状腺过氧化物酶抗体水平,同时测定甲状腺球蛋白抗体作为比较。结果原发性甲状腺功能紊乱患者血清甲状腺过氧化物酶抗体水平为(213.5±143.5)IU/mL,桥本甲状腺炎患者血清甲状腺过氧化物酶抗体水平为(241.5±163.7)IU/mL,Graves病患者血清甲状腺过氧化物酶抗体水平为(259.8±188.5)IU/mL,志愿者对照组为各组(17.2±11.5)IU/mL,患者数据均显著高于对照组,数据差异有统计学意义(t=7.495、7.488、7.038,P<0.05),同时,原发性甲状腺功能减退、桥本甲状腺炎、Graves病患者血清甲状腺过氧化物酶抗体随病情变化有升降;甲状腺球蛋白抗体与血清甲状腺过氧化物酶抗体之间为正相关关系。结论血清甲状腺过氧化物酶抗体可以作为自身免疫性甲状腺疾病诊断的一个重要参考指标,其特异性较高,可以在临床中进行联合推广应用。

Objective To evaluate the value of serum thyroid peroxidase antibody in the diagnosis of autoimmune thyroid diseases.Methods A total of 78 patients with autoimmune thyroid disease treated in our hospital were randomly selected as the study subjects.The number of cases was selected from August 2016 to December 2018,including 20 patients with primary thyroid dysfunction.29 patients with Hashimoto's thyroiditis,29 patients with Graves'disease,and 30 healthy volunteers who were examined in our hospital as a control group.Serum thyroid peroxidase antibody was compared between patients with autoimmune thyroid disease and control group.Levels were simultaneously measured for thyroglobulin antibodies as a comparison.Results The serum thyroid peroxidase antibody level in patients with primary thyroid dysfunction was(213.5±143.5)IU/mL.The serum thyroid peroxidase antibody level in patients with Hashimoto's thyroiditis was(241.5±163.7)IU/mL.The thyroid peroxidase antibody level was(259.8±188.5)IU/mL,and the volunteer control group was(17.2±11.5)IU/mL.The patient data were significantly higher than the control group.The difference was statistically significant(t=7.495,7.488,7.038,P<0.05).At the same time,serum thyroid peroxidase antibody in patients with primary hypothyroidism,Hashimoto's thyroiditis and Graves'disease has a rise and fall with changes in the condition;thyroglobulin antibody and serum thyroid peroxidase antibody.There is a positive correlation between them.Conclusion Serum thyroid peroxidase antibody can be used as an important reference for the diagnosis of autoimmune thyroid diseases.Its specificity is high and can be promoted and applied in clinical practice.
